Employee reported that their eye was irritated after making a cable splice. Employee was unable to identify the mechanism of injury, but was diagnosed with having a foreign object in their eye that was removed in an outpatient surgery.

Employee was using a sledgehammer to pound a hard bar pin out from a dozer roller frame. A piece of metal broke off from the hard bar pin and then struck the employee over EE's right eyebrow. Employee had the metal removed 37 working days after incident occurred. Employee received 6 sutures. 6 sutures

Employee was injured when EE struck self in the face with a ratchet while tightening the gear case mounting bolts on a continuous miner causing a laceration above EE's left eye resulting in 18 sutures.

A motorman at Maple Eagle #1 injured right pinky finger while attempting to put the 15-ton Brookville locomotive back on the rail. EE grabbed the handle of the lifting jack to adjust its position on the mine floor and had not removed their hand. When the weight of the locomotive was applied, this pinched the victim's pinky finger between the jack handle and lifting mechanism.

Employee was in the process of knocking the miner boom pin back in on unit #2 on #112 C/M, when a piece of metal came off the pin and struck the employee in the left pinky finger. On 04/25/2024 the employee had the metal taken out of EE's finger at CAMC and was placed off work.

Employee was using a wrench hand tool to loosen up a nut threaded on a bolt. Employee stated when the nut broke free the employee's momentum took their hand forward resulting in left pinky finger being caught between the wrench and guarding. Employee's left pinky finger was cut and broken.

Teammate stated EE was in the process of replacing bits on the 088 miner. EE had removed the old bit and was pushing the new bit into place when it became stuck so EE took EE's pick hammer and struck the bit to force it in place. After several hits a piece of metal came back and struck EE. On 3/30/2023 EE had the metal removed and received 3 stitches.

EE stated: was skiving a piece of belt to put a splice on it. In the process of of cutting the belt with the Stanley utility knife, the knife slipped out of the belt and punctured EE's left hand requiring 2 stitches.

A contract motorman working through a contractor company was in the process of jacking a flatcar back on the track, when the motorman slipped and fell on right knee resulting in pain in lower back. The motorman sought treatment at a clinic where the motorman was placed on restricted duty.

EE stated that EE was in the process of squaring on the end on the belt to make a belt splice during a Pull Back. EE stated that the belt was laying on a board, that was laying across the belt rails and that while EE was cutting from the middle of the belt towards self that the knife slipped, and EE cut self across the right thigh. The employee received 5 stitches

Employee was cutting pieces of rubber off a screen using downward force with a razor knife. Employee had a rubber piece to tear and the downward momentum resulted with the employee striking right leg with the razor knife. Employee had a laceration and was treated at a local facility that administered treatment with 7 sutures.

Employee stated was in the process of adding 4" aqua waterline in prep for a belt move. Employee picked up a crib block and used it to strike the end of the waterline and after several hits employee caught a finger between the crib and the waterline. On December 7th employee had the end of the finger removed due to nothing left to protect the bone.

Employee was changing a rotation hose on the 419 roof bolter and the channel locks employee was using slipped off the fitting causing employee's right palm to come in contact with a metal tag on the bolt tray. This caused a 5 inch cut that required 5 stitches.

Employee stated that EE was using a sledgehammer to knock a stopping when the injury occurred, Employee was knocking a stopping and the employee was "choking up" on the hammer handle while swinging into a block stopping causing EE's finger to be caught between the stopping and the hammer handle causing a laceration below the knuckle on employees left index finger

Injured employee was lowering a motor using a 1 1/2 ton chain ratchet and unlocked the ratchet to use the wheel to lower the motor. When the motor began to fall, the employee grabbed the chain pulling EE's hand into the ratchet resulting in a laceration that required 7 sutures and fractured the tip of the left index finger. No Lost or Restricted Workdays.

Employee stated that they were working on the parallel A belt motor reducer. EE was holding a wrench in place while another employee tightened the bolts. The wrench slipped off the bolt and smashed EE's right ring finger between the wrench and the frame of the belt head.

Employee was in the process of changing a tie rod end on a shuttle car. The rod end was seized so EE put right arm against frame of shuttle car to increase force on wrench. While doing so EE felt a pop and instant pain in right shoulder. This employee returned to work the next day and recently went back to doctor and was placed off work for surgery as of 5/18/2018.

Employee was attempting to hang a light on the #70 long wall shield using zip ties. While doing so EE was attempting to cut off the excess zip tie with a razor knife and slipped and cut the left hand. Employee received stitches to close laceration.

Employee stated there was a rock fouled underneath the chain of a shuttle car. In order to remove it employee was striking the pin out that holds the flight in place. In doing so the flight broke free and struck employee in the face causing a laceration that required several stitches.

Employee was installing a high voltage plug in the back of the section power center. EE was having trouble tightening the plug all the way and EE struck the high voltage plug wrench with a hammer to assist in tightening it. EE accidentally hit hand between the thumb and pointer finger. EE received two stitches.

Employee was assisting install a new set of air lock doors on the 20 HG longwall section. EE set the jack against the mine roof and was preparing to adjust the set screws to make the jack secure firmly to the mine roof. In the process of preparing to tighten the set screws the jack slipped and slid down smashing the employees left hand and finger.

Employee was trying to take a shield off the reel box on the #322 shuttle car on the 1 North Section. Employee was using a 3/4" inch wrench to try to break the bolt loose. EE was pulling the wrench toward self when this incident occurred. In the process of pulling on the wrench it slipped off causing EE to fall on the ground landing on right hand.

Injured EE was assisting another EE to change out a planetary on the #218 C/M machine in use on 1 North Section. EE was prying down on planetary with a slate bar while co-worker was striking it w/16lb sledge hammer. In the process of co-worker striking planetary w/sledge hammer a piece of metal flew off of the planetary & struck EE in the face causing puncture wound.

Employee was assisting another employee change the conveyor pot on the #216 C/M machine. The employees were kneeled down in the pan of the C/M machine using slate bars to try to line the conveyor pot holes up to be mounted to the machine. In the process of EE pushing the conveyor pot forward with a slate bar EE felt a pain in EE's stomach at EE's abdomen.
