The employee was prying down a loose hanging above the continuous miner in 9 North Mains. When the rock broke free, the slate bar came down and contacted employee's left ring finger. The employee received 10 stitches and a fracture to the end of the finger.

The employee was holding a piece of 8 inch pipe while another employee was cutting the pipe. The pipe was shifting and moving while cutting so they added a chain hoist to stabilize the pipe. The employee continued holding the pipe with the chain added and the pipe rolled pinching the employees left thumb between the pipe and chain.

EE was using a slide hammer removing the TG ranging arm pin, when the pin let loose it caught EE's right index finger between the pin and bill of a 636 scoop.

The employee was working on 9 North Mains Section. EE was on top of the miner breaking up a rock that had the conveyor chain fouled. When the employee struck the rock, the handle of the sledgehammer struck EE in the chin, causing EE to hit head off the mine roof. This resulted in EE biting through EE's tongue. The employee was trained and did not violate any laws.

The employee was at the Harvey Run Hoist doing inspections. While the employee was checking the brakes, employee felt a pop in left shoulder. The employee did not violate any laws or company policies and is trained on this job task.

The employee was repairing a shuttle car cable. While cutting the old splice away cut EE's left index finger. The employee received 8 stitches.

The employee was prying a cover off of a stage loader crusher, that was being held up by come-a-longs. When the come-a-longs got the full weight of the cover, the come-a-long came loose from the top and the employee was caught between the crusher cover and a pile of cable laying on the pavement behind EE. The employee was wearing all the appropriate PPE.

Contracting employee was cutting belt with razor knife, the knife slipped and contacted upper leg requiring stitches. Cause of this incident was body positioning and paying attention. Compliance with company rules a factor, mining/equip, PPE not a factor. Miner proficiency may be a factor.

Injured was using a sledge hammer to create a hole in a return stopping for a longwall air change. The employee was attempting to control the sledge hammer with non-dominant hand and lost control, which caused employee to hit finger against a mandoor, resulting in sutures to left index finger.

Clearing material from the floor of 13 belt at 134 block (in front of belt cleaner)and was cutting up a 4" hose with a utility knife when the knife slipped and employee stabbed themselves in the left upper thigh just above the knee, resulting in a laceration that required 4 sutures to close.

While cleaning 1 North track haulage (around 13 block) with a mattock, employee hit one of the rails with the mattock and experienced pain in the lower back. Employee finished the shift, but went to the doctor afterwards and was given time off due to a strain of the lower back.

Injured was attending the 16 belt (plant feed) belt transfer at the preparation plant. A rock got stuck in the grizzly bars and employee was using a rake to remove the rock. While pulling on the rake employee felt pain in employee's neck and left shoulder area, and was later diagnosed with a cervical and thoracic strain.

Injured was in maintenance trainee program and was making a splice on 24 Shuttle Car cable in St Leo rehab area. While hitting beat block with bit hammer (to crimp wires together), the hammer glanced off and struck EE's left thumb resulting in a fracture of the distal joint in the phalange and a laceration that required 9 sutures to close.

Holding plug on jumper cable while two others hit it with hammers to separate the jumper cable plugs (3E longwall). Metal shard came off hammer and hit EE in abdomen. Persistent discomfort caused EE to seek medical attention, which discovered a shard of metal embedded in EE's abdomen. Started missing work 8/1/17 to have the shard surgically removed.

Injured was putting rubber matting over leads on B56 motor, while it was located in the main wash down bay of the Metz Motor Barn. While cutting holes in the rubber matting with a utility knife, the knife slipped and EE stabbed self in the left palm, in the meaty part below the thumb. Required 4 sutures, employee missed no work due to this injury.

The injured employee and two others were using hammers and speed loaders to install rivets on a mechanical splice. As another person was swinging a hammer to seat rivet, injured employee reached to reposition the speed loader and was struck on the back of the right hand.

Putting conveyor chain back on miner in St Leo rehab area, injured held pin while another miner drove it in with a sledge hammer. Hammer glanced off chain and struck injured in mouth, resulting in a small laceration on lip and chipping right upper incisor. No problems with PPE, training, mining methods or compliance issues.

Crawler Mobile Tailpiece (CMT) cats were stuck and EE was attempting to free them with assistance of 3 ton chain hoist. The tail hook slipped off the CMT cats and flew off under tension, striking EE in the upper lip, resulting in a laceration. Received 23 sutures. No problems with mining methods, training, PPE or compliance issues.

Removing belt structure at 2E longwall tail, using a hammer to beat out a pin in the structure, with EE's head positioned under the structure. A piece of debris (metal/rust/dirt) fell, bounced under EE's glasses and landed in right eye, resulting in a corneal abrasion. No problems with mining methods, training, PPE, or compliance issues.

The injured and another individual were replacing bolts on the clarifying pump. As the employee rolled the first bolt on, ee used a pipe wrench to turn the driven sheave. When the bolt rolled, the pipe wrench spun around catching finger between the wrench and a crib block. No problems with PPE, mining methods, training or compliance issues.

Making splice on 3East belt, miner was just inby drive trying to remove a stuck splice pin. EE put the claw of a hammer against the head of the pin and struck it with another hammer. Sliver of metal came off one hammer and punctured left forearm. No problems with training, PPE, mining methods or compliance issues.
