The employee was lifting hoist chains above EE's shoulders to hook to a forklift when EE felt pain in EE's right shoulder. The employee was using first-aid measures until the pain continued and then was prescribed physical therapy sessions which were started on 10/15/2020.
Mine employee was injured while securing an impact plate on a feeder chute. The plate was secured in place by the employee using a pry bar. The pry bar slipped allowing the plate to shift coming into contact with the employee's left index finger causing a laceration that required four sutures.
