Employee using breaker bar to help customer truck driver close bottom gate. As employee was exerting force employee rolled right ankle resulting in a spiral fracture of the right ankle. Employee was wearing muck boots at the time which did not offer the support needed for job being done.

Showing all 4Employee was using EE's personal knife to cut a zip tie and cut EE's left thumb. EE was not wearing gloves, and was pulling the knife towards EE's body. When EE cut through the zip tie the knife struck EE's left thumb. This resulted in an approximately 1 inch laceration which took 6 stitches to close laceration.
Injury was not reported to company until Tuesday July 27, 2021. Employee was installing a speed bump at the Load Out area. While kneeling down on the right knee EE began to swing hammer side to side to loosen stake to remove from ground. EE allegedly hit left knee with hammer resulting in a cellulitis infection in EE's left knee.
Employee working on bucket elevator 306 heating a bearing with torch trying to wedge out of place. Employee hit with hammer and was hit by piece of shrapnel on right hand across knuckles causing laceration. Laceration took 7 stitches to close laceration.
