Employee was tracking a conveyor belt on the return side of the conveyor using a small sledge hammer to adjust the return rollers. Employee missed EE's target and smashed EE's middle finger tip, resulting in a broken finger tip.

Showing all 3Employee was changing a tail pulley bearing. The all thread part of the take up was ceased up. Once the all thread broke free, while using a break away bar, the employee's left middle finger was mashed between a lifting eye and break away tool. Resulted in a laceration and broken finger tip.
The employee was cutting tape off of motor leads to the disconnect fan. EE then cut the right thumb to the back side of EE's hand. The cut required stitches.
