IE was setting a timber in the beltline. A piece of scale rock fell when EE was hammering in wedges striking the employee on the back of EE's head/neck area causing a laceration. IE received two staples to close the wound.

Employee was rolling up conveyor belt at our surface location with a belt winder. The winder came loose striking the IE on top of hard hat causing laceration to the head area.

IE was putting a 4" pipe together using a hammer. IE was swinging hammer towards EE's body and missed the pipe striking EE's left shin causing bruising.

EE was struck on top of the right foot while in the process of installing a straight bottom roller in a high area. The employees were using chain hoists to lift the roller into place when the chain slipped off allowing the roller to fall.
