Showing all 6Employee was using a 36" pipe wrench to close the isolation wheel valve of DWS108. EE had the pipe wrench placed on the top of the wheel at the 1:00 position and began pushing the pipe wrench counterclockwise to close the valve when the pipe wrench slipped off of the wheel causing EE's left pinky finger to get smashed between the pipe wrench handle and the adjacent I-Beam.
Employee attempted to lower the suspended counterweight. The chainfall failed causing the counterweight to fall & strike the employee in the right shoulder.
Employee was attempting to remove a large nut on SCR 311 in order to adjust the hanger arm with a 36" pipe wrench. Nut would not break loose so second employee came in to help. Both employees were pulling on pipe wrench when nut gave way, causing second employees thumb to be bent backwards, injuring tendons in thumb.
Employee clocked in and returned to personal car to retrieve back pack. Using a pocket knife, employee was attempting to cut a zip tie off back pack. Knife slipped cutting left index finger. Initial first aid was preformed, however, bleeding continued and employee was transported to ER where stitches were received.
EE was cutting a zip tie with pocket knife and cut EE's left thumb requiring 3 stitches
