Employee was using a pick to cut a coal sample from the rib. While swinging the pick into the rib, a small piece of coal flew back and hit EE in the mouth, causing a small cut to EE's upper and lower lip. Employee received one stitch in EE's lip. Returned to work EE's next shift.

Employee assisting laying track rails on the 7 North Haulage. EE was helping co-workers roll rail over using a long pry bar. As they were rolling a rail, EE stopped but employees on the other end continued to roll their end, causing the bar EE was using to swing back and struck EE on the right side of EE's neck and jaw. Failed to maintain control of the hand tool EE was using.

Injured employee was using a rail hook to lift a track rail and felt a "pop" in their back. They then stood up and stumbled and fell on right hand smashing their index finger. Employee sought treatment the following day for back pain. No rules or regulations were violated.

Employee was installing a ram pin at 148 shield on the 7W Longwall face. EE was using a hammer to drive the pin in. EE switched hands to hit the pin squarely. As EE was striking the pin with the hammer EE missed and hit EE's right little finger, causing a laceration requiring 7 stitches to close. Employee has training & experience and was wearing required PPE.

Injured employee was using a small "hook chisel" to remove a staple from a hose fitting at #74 Shield on the 7W Longwall face. As EE was prying the staple out, it broke free and the chisel came up and struck EE's face causing a chipped tooth. Employee did not lose any workdays due to this accident but had the tooth repaired on 4-23-25. Has training and experience.

Employee was using a small sledge hammer to remove a block from a stopping at a regulator for the purpose of installing a de-gas pipe through the stopping. EE was holding the hammer with left hand and as EE was striking the block wall, EE smashed their hand against the metal frame of the regulator and the hammer handle causing a cut and fracture to the tip of EE's left thumb.

Employee was using a knife to open a box of belt rivets. While cutting, the knife slipped and cut employee's right hand below the thumb, palm side. Received 4 stitches. Employee failed to maintain control of the knife and was cutting toward self. Employee was wearing protective gloves. Returned to work the next shift.

Injured employee was using a hammer and chisel to remove a plug in a longwall ram jack that needed replaced. While hitting the plug with the hammer & chisel, a piece of it broke off and flew into employee's left eye. Received a cut to the globe part of EE's eyeball that required sutures. Employee has training & experience but is new to this job.

Employee was using a knife to cut a zip-tie off a hose hanger supporting hydraulic hoses. EE had left hand on the hose holding it in place while they cut the zip-tie away. As they cut the zip-tie away, the knife contacted left hand cutting the top of middle finger. Received 3 stitches to close the cut. Accident caused by employee's failure to use a knife in a safe manner.

Injured employee & co-worker were putting a Loading Machine conveyor chain together. Injured employee was holding the connecting link in place while co-worker was driving link in place with a hammer. Co-worker missed with hammer and struck injured employee's right little finger causing a laceration requiring 6 sutures to close. Employee has training & experience.

Employee was using a pry bar to remove a rock off the top of the continuous miner. As employee was prying on the rock the pry bar slipped out from under the rock causing employee's left elbow to strike the mine roof. Has pain & soreness in elbow. Employee didn't violate any rules or regulations but failed to maintain control of the hand tool the employee was using.

Employee was using a ratchet & socket to loosen a bolt on the infrared shield initiation sensor mounted on the longwall shearer so EE could adjust it's position. As EE was applying pressure with the left hand to the ratchet handle, the bolt broke loose causing EE's left arm to suddenly move forward causing pain in the left shoulder. Employee has training & experience.

While removing belt structure on 3W belt conveyor, employee swung a hammer to hit a bedrail pin, missed, and hit their thumb. This resulted in a bruise to the left thumb. Employee was wearing required PPE and didn't violate any rules or regulations.

Injured employee was using a steel rod to knock loose rock that had caused a rock jam in the 2E longwall panline. As employee was jabbing the rocks with the bar employee smashed a finger between the bar & rock/pan. Employee received 2 stitches to the tip of right middle finger. Returned to work next shift. Employee didn't violate any regulations & was wearing required PPE.
