IE was working with another employee to offload a steel set parts crate from an irwin car. They were using lever bars to move the crate. The other employee's bar slipped and all of the weight was suddenly on IE's bar. This caused IE's finger to be caught between the bar and the elevated portion of the car's deck. Injury was not the result of a training or compliance issue.

Injured was cutting a zip tie off an oil soaked hydraulic hose in the Metz Motor Barn washdown bay when EE's left hand holding the hose slipped off and EE struck EE's left index finger on the razor knife blade resulting in a laceration that required 4 sutures to close. While trying to loosen a stuck dust hose fitting with a pair of channel locks on the twin boom bolter in St Leo Mains at 105 block they felt pain in lower back. No problems with equipment or compliance issues, training or PPE.

While using a pry bar installing new track ties near 106 block 9 South they felt a tear/pop/pain in the soft tissue of upper left thigh, possible muscle strain. X-rays negative, referred to specialist for possible MRI. No problems with equipment or compliance issues, training or PPE.
Injured was cutting out exposed roof bolts around 106 block Main West haulage with a band saw when the bolt shaft let loose sooner than expected causing EE to swing the hand tool into EE's off hand, resulting in a laceration to EE's right index finger that required 4 sutures to close. No problems with compliance or equipment issues, training or PPE.
Employee was cutting airline when lost control of the airline and caused the knife employee was using to contact left index finger. This resulted in a laceration that required three sutures. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
The EE was helping unload double stacked belt sleds from a duck bill scoop. The winch hook became fouled, and wouldn't allow the sleds to slide of off the bill. The injured struck the hook with a sledge hammer, as EE did this the sled dropped. The sleds pinched the EE's left foot between the bill and sleds. Began missing 5-16-24.
The injured was cutting a piece of dust hose. As EE was cutting away from self, the knife slipped and EE cut left hand between the thumb and index finger that was holding the hose in place. EE finished shift, began missing 4/17/24.
The EE was assisting the changing of inserts in the CM head. When one of the inserts became stuck, the EE struck it with a hammer. A piece of the insert broke off and struck EE's left bicep. EE worked until 1/2/24, when EE was put off.
The EE was using a razor knife to remove tape from a drill steel. The injured did not close the blade, and their left pinky and middle finger contacted the knife. The lacerations required 6 stitches. The EE was wearing all appropriate PPE at the time of the accident, and did not miss any work.
The EE was making a cable splice on the FCT, when the blade slipped causing EE to cut left knee with the utility knife. The EE was wearing all PPE. Began missing work 5-18-23, Returned 5-22-23
The EE was using a utility knife to cut down ropes holding up the loader cable. The knife slipped and the injured cut EE's left middle finger. The EE was wearing gloves. EE began missing work 3-22-23, Returned 3-24-23.
Injured was attempting to move a loose rail with a track bar and the rail rolled down the bar causing the bar to strike their right knee on the left side. No problems with equipment or compliance issues, training or PPE.
The EE was assisting the miner operator changing bits on the machine. As the injured was placing bits in the head, the hammer slipped and struck the EE's right thumb. Began missing work 1/19/23.
While using a lever chain hoist on the 4 West beltline (while installing belt) the lever slipped and they felt like they pulled something in right elbow. Started missing work 1/13/23 for lateral epicondylitis of right elbow. No problems with equipment or compliance issues, training or PPE.
The EE was digging out material from under a set of haulage doors. The injured raised up on the handle of the pick and felt a pop in lower back. The EE did not finish the shift, and began missing work on 12/13/22.
The EE was working in 2W Longwall teardown. The injured was using a socket and wrench to remove bolts from old pan line. While doing so, EE felt pain in the left shoulder. EE worked till 8/4/22, then began missing. RTW on 8/8/22.
Employee was using a come along to install a head roller bearing cap when EE pulled down on the lever to lift it EE experienced pain in EE's right wrist.
The EE was taking apart a 2" staple lock hose from the old power center location. The EE was using a hammer to take apart the hose at the joint. As the EE struck the joint, the hammer glanced off striking EE's right middle finger. EE began missing 6-1-22.
The injured EE was using a utility knife to cut a zip tie. The knife slipped and the EE lacerated left wrist. EE received four stitches.
While making a splice on an electrical cable the injured missed the beat block with a hammer and struck EE's own left thumb with the hammer, resulting in a mildly displaced avulsion fracture at the tuft of the first distal phalanx. No problems with equipment or compliance issues, training or PPE.
The injured employee was rib pinning in the 9 North Mains 3 to 5 crosscut. The injured pried down a rib that was approx. 5' high 6" thick. As the rib fell it broke and a piece struck the employee in left knee. Employee began missing work 08/18/2021.
The EE was working on 2 West Belt Line, around the drive area. The injured was pulling a piece of belt with a pair of channel locks, when EE lost EE's grip and the channel locks slipped off. This caused the EE to fall back into a timber that was laying on the pavement, injuring EE's lower back and left hip.
The injured EE was setting L/W handrails with a chain hoist. As the EE was raising a hand rail into position, the handrail fell striking EE on the right thigh and foot. The injured didn't sustain any fractures, but was put off work until 6/28/21.
The employee was outside in the lamproom at the Miracle Run Portal. The employee was cutting apart a new pair of gloves with a razor knife. The knife slipped and employee cut left middle finger. Received 5 stitches.
