An employee was cutting a zip tie off of a D-ring when the knife slipped and sliced the inside of EE's left forearm. The laceration was approximately 1" in length and required 3 stitches. The employee was permitted to return to work with instructions to keep the wound covered. This incident did not result in any lost time.

Employee was using a knife to cut mesh. Employee was cutting toward themselves and slipped, causing the knife to penetrate their right thigh leading to a laceration. Employee received 2 stitches to close the wound.
A seasonal worker was using an unapproved box cutter to cut up a bag when they cut their hand. The cut required stitches.
Employee was hammering a collet pin out of a boom swing pin, missed the pin and smashed middle finger.
An Matco Electric contractor was using a post pounder to drive rebar into the ground at the #4 shaft location. Upon lifting the post pounder up, the EE lifted to high causing the rebar to come outside the pounder. When the contractor came down with the pounder the rebar pinched their left thumb between the handle and the top of the rebar. Broken bone/stitches to thumb.
A contractor received a laceration to the back of their hand when their 1 & 13/16" wrench slipped off of a nut. This resulting in their gloved hand striking a threaded stud nearby. The contractor required stitches for the cut.
The employee was removing a guard. EE was holding the guard with left hand and struck the guard with a hammer with right hand. The guard dropped down & pinched the small finger on left hand between a block and the handle on the guard. The employee broke small finger on left hand.
A maintenance employee was using a lock back utility knife to remove the lid from an oil bucket. While in the process of cutting the lid free, the employee's hands slipped and the blade of the utility knife contacted the back of ee left thumb. The blade cut through ee glove and caused a small laceration.
Employee was making holes in an old belt used for skirting in NW2B/NW2A transfer. Employee was wearing gloves and used a hole punch with a 3 lb hammer to cut holes. Employee was in the process of driving the punch through the belt when the hammer struck their left thumb. Tip of the left thumb was broken.
Employee was cutting belt line with a retractable knife. Employee put themselves in the line of fire by cutting towards wrist and cut themselves needing sutures.
An employee was using a retractable blade safety knife to cut a tie wrap. EE had the safety knife in right hand. EE was holding a pipe and hose in left hand. The pipe moved and the knife stuck the employee's left thumb causing an approximate 2" laceration on the inside portion. The employee was taken in for medical evaluation, received sutures and released to full duty.
Employee was cutting the banding off of troughing idlers. When the final band was cut the first row of idlers fell off of the pallet striking the employee on the right foot, penetrating the boot behind the steel toe and injuring right big toe. The employee fell to the ground between the stack of idlers and a material handler which was parked aproximately 4' away.
An EE was making a vacuum hose for M-27 Bolter in the Smag Bay. The EE was holding the hose with their left hand and cutting with their right. The force needed to cut the hose to length caused the knife to go all the way through, but sliced the EE's pointer finger at the same time. EE was not wearing gloves during this job. The EE received 5 stitches on their left pointer finger.
An employee was cutting a 1" flex pipe with a safety knife. The knife slipped and cut through the black dot glove and the inside of his middle joint on the middle finger of his left hand. The employee needed sutures to repair the laceration. He was also given a prescription for the pain. The employee was released to full duty on his next shift with no restrictions.
While attempting to shift approach rail away from the #6 scale rail, the joint bolts were removed and a steel wedge was driven into the gap to force joints to tighten the gap that opened up in the joint. The new bolts were installed and the wedge was hit with a 10-lb. sledge hammer. The wedge jumped out and flew into EE's leg causing the injury to occur.
TWO MECHANICS AND A SUPERVISOR WERE ATTEMPTING TO RAISE THE TRIPPER BRIDGE TO MAKE AN UPGRADE. WHILE THE SUPERVISOR WAS ASSISTING THE TWO MECHANICS BY OPERATING THE MANUAL JACK, MOVEMENT IN THE JACK ARRANGEMENT OCCURRED CAUSING THE MATERIAL USED AS AN EXTENT ION BETWEEN THE JACK AND THE TRIPPER BRIDGE TO BE THROWN OUT. THE EXTENSION STEEL STRUCK THE SUPERVISOR IN THE HEAD.
Employee was using a 2" ball valve to try and drive out a pin to change out forks on a Gradall Material Handler. The employee missed his target and sliced his left pinky finger open requiring 3 stitches.
Employee had no particular point during his shift where he felt any pull or pain. He noted that he was pulling up on a wrench all night while changing clips in the production shaft. He went straight to sleep after leaving work and woke with the pain. He has been diagnosed with a incarserated hernia that will be surgically repaired on Thursday January 21st.
Employee was replacing bolts in gallery structure. Driving old 3/4" bolts out of wood structure using new 3/4" bolts as a punch. As he was striking bolt with sledge hammer, his left elbow struck an existing bolt. Bruised left elbow.
While dismantling exhaust equipment to facilitate the removal of an engine, a socket wrench slipped from the nut. This caused the employee's elbow to move backwards allowing it to strike a piece of angle iron with sufficient force to make a small cut above the left elbow, requiring stitches.
EE was in the process of repairing a belt splice. When the belt was placed into the Flexco 840 belt cutting device, the blade was not in the fully retracted position, which exposed the cutting blade. EE's hand slipped and came into contact with the cutting blade, causing a laceration at the pinky joint.
Employee was training on a conveyor belt. He struck the structure at the base of a troughing idle roller, and as he pulled the hammer back it contacted the moving conveyor belt. His index finger was pinched between the roller and the conveyor belt. This resulted in lacerations to the tip of the finger. The injury requird 13 sutures.
While working on communication equipment, EE relaxed his hand to his side where his right pinky finger came into contact with the partially exposed blade of a utility knife. Medical aid was sought and five stiches were required.
While on break employee used knife to cut chocolate. When attempting to close knife, lock mechanism (located at the rear of this model) stuck and employee pulled harder on the back of the knife causing him to lose his grip, and the knife to slide in his grasp. The blade slid along his thumb. The laceration required five stitches to close.
Employee was cutting a hole in the plastic cover of a golf cart with a hawk bill knife. The employee was in the process of securing the cover to the cart. The knife cut through the plastic cover and continued forward slicing employee's left index finger. Employee was taken to medical facility and wound was closed sutures.
Employee was changing a drill bit on SMAG BW-50 drill. He was using a rock hammer (geologist pic) to remove bit. The hammer glanced off bit and struck employee between thumb and forefinger, puncturing left hand. Required 3 stitches, returned to regular duties.
A 1900 LB PLC CABINET HAD TO BE BROUGHT INSIDE & INSTALLED ON A 3" HIGH PAD. THE CABINET WAS IN AND BEING MANEUVEREED ONTO THE PAD. EE HAD A PRY BAR TO LEVEL 1 CORNER UP AT A TIME. THE CABINET SHIFTED AND CAUSED THE LEVER TO MAKE A QUICK DOWNWARD MOTION THAT CAUSED EE TO STRAIN A MUSCLE IN HIS BACK.
Employee was cutting plastic bands on a pallet of stretch wrap using a locking blade knife. As he cut one of the straps, his motion carried the knife toward him and the knife struck his left leg just below the knee. Employee received 2 stitches.
Employee was attempting to lift a new pump stand to turn over to paint. Used small hoist. As he lifted the stand off the table, the chains came off the end of the stand and pinched his right hand between table and stand. Right index finger: three stitches, right middle finger: 2 stitches. Restricted duty 10 days.
EE WAS ATTEMPTING TO CUT OUT A TRIANGLE PIECE OF A CONVEYOR BELT. HE CUT THE FIRST TWO SIDES OF THE TRIANGLE AND PULLED THAT PIECE BACK SO THE THIRD SIDE WOULD BE EASIER TO CUT. AS HE CUT TOWA RDS HIMSELF WITH A UTILITY KNIFE, HE WAS UNABLE THE BLADE BEFORE IT CUT HIS ABDOMEN. HE RECEIVED THREE STITCHES ON THE SAME DAY OF THE ACCIDENT, MARCH 4, 2004.
AS EMPLOYEE COMPLETED HIS WORKPLACE INSPECTION PRIOR TO ENTERING THE TUNNEL TO POWDER, HE NOTICED SCALE ON THE RIB. HE THEN ATTEMPTED TO REMOVE THE 4' BY 4' SCALE BY HAND SCALING WITH SCALING BAR. DURING THE REMOVAL OF THE SCALE, THE SCALE CAME LOOSE. AS THE SCALE FELL, IT KNOCKED THE SCALING BAR OUT OF HIS HAND AND THE SCALING BAR STRUCK THE EMPLOYEE BEHIND THE LEFT EAR.
EMPLOYEE WAS EXITING CREW TRUCK, WHEN HIS RIGHT HAND CAME IN CONTACT WITH HIS UTILITY KNIFE. THE KNIFE WAS HANGING FROM A KEY RING ATTACHED TO HIS MINER'S BELT. THE BLADE HAD PARTIALLY OPENED , CAUSING A CUT TO HIS RIGHT PALM.
EE WAS TIGHTENING A LUG ON HIS TRUCK TIRE ON THELOWER SALT PAD WITH A SOCKET. THE LUG AND STUD BROKE WHILE HE WAS PULLING DOWN ON THE SOCKET. HE WAS STANDING UP, BUT THE FORCE MADE HIM SQUAT STRAIGHT DOWN LANDING ON HIS R. ELBOW THAT WAS BENT AT A 90 DEGREE ANGLE, PUSHING HIS RT. SHOULDER UP.
TWO EE'S WERE INSTALLING A BELTLINE DRIVE AND TAKE-UP UNITS. THEY WERE TRYING TO LINE UP THE 2 FLANGES OF THE DRIVE AND TAKE-UP. AFTER PRYING UNITS TOGETHER, INJURED EE TRIED TO INSTALL BOLT. THE BOLT WOULD NOT FIT. HE SAID TO STOP PRYING SO HE COULD CHECK THE ALIGNMENT. HE THEN CHECKED TO ALIGNMENT WITH HIS FINGER. THE OTHER EE NEVER HEARD STOP ORDER, CONTINUED PRYING. 8 STITCHES
EMPLOYEE WAS USING A HAMMER TO DRIVE A CONNECTING PIN OUT OF AN UNDERCUTTER CUTTING CHAIN. THE HEAD FLEW OFF STRIKING THE EMPLOYEE'S LEFT HAND, CAUSING A LACERATION TO THE MIDDLE FINGER. REQUI RED 5 STITCHES. HAMMER WAS DEFECTIVE. HAD PREVIOUSLY BEEN REPAIRED WITH A MAKE-SHIFT WEDGE.
