THE EMPLOYEE WAS TIGHTENING THE BEARING BOLTS ON FINISH MILL 4, ELEVATOR 1. THE WRENCH HE WAS USING SLIPPED OFF OF ONE OF THE BOLTS CAUSING THE EMPLOYEE TO SLIP AND FALL TO THE FLOOR GRATING. HE STRUCK HIS LEFT KNEE ON THE GRATING.

WHILE LIFTING A PRIMARY SCREEN HOUSING WITH 2 PULL LIFTS, A 3-TON ON THE WEST SIDE AND A 11/2 TON ON THE EAST SIDE OF THE SCREEN HOUSING, INJURED EMPLOYEE WAS PULLING UP ON THE HANDLE OF 11/2 TON PULL LIFT. THE HANDLE BROKE OFF FROM THE PULL LIFT CAUSING THE EMPLOYEE TO LOOSE HIS BALANCE AND FALL BACKWARDS STRIKING HIMSELF AGAINST A GUARD RAILING.
