Showing all 48Employee was cleaning the area behind the crusher drive belt and was using a screw driver to scrape the rock out. The screw driver slipped, struck his left thigh puncturing it.
Employee was cutting the straps off a new pallet of empty bags. He was momentarily distracted and cut his left index finger requiring sutures.
Employee was stripping a 5kV cable when the knife slipped cutting his left index finger. Sutures were required because the cut was over a joint.
Mechanics were removing a pipe, when they cut the bolts supporting it the temporary support allowed the pipe to twist and fall striking the employee on the head, shoulder and back. The employee received a slight concussion.
Working on closing discharge to south set of sluice pumps. Holding a wrench & was struck in the hand with a sledge being used by co-worker.
Replacing 6" knife gate on y-9 slurry line. While trying to bolt the valve back in was using a 24" pry bar trying to align the holes. In prying with the bar it slipped striking right cheek causing a 1" laceration.
Tightening bolt on side rotor clutch using open end wrench. Started to tighten bolt; as it got tighter the open side of the wrench started to flex open & let go of the bolt head. Downward pressure resulted in wrench striking the miner on the chin causing laceration on right side of chin.
Rotating pot & valve on underflow of pump. Tight & confined space. Had to reach around valves to get to bolts to tighten them. While he was doing this he felt his shoulder getting very sore. The 7000-1 is being filed now because the Safety Dept. became aware on 7/21 that he was unable to do his regular job tasks.
Using a pipe wrench to close a 4: block valve; pulled on wrench & heard a pop & felt grinding & pain in his Right Shoulder
Employee was using alignment bar to align pipe flange. Middle finger of right hand contacted sharp edge on pipe insulation lagging resulting in 1 1/2" laceration on side of finger between first & second knuckle.
Employee was reseting a crusher when the wrench struck the top of his right hand causing a boxer fracture.
Employee was checking for bent shaft on CB-2115 belt drive end. Placed alignment bar on rotating drive pulley shaft; bar came in contact with key way & kicked back striking employee in the face.
Cutting tabs on conveyor belt & inserting them into belt core. Kneeling on the belt, had both hands on the knife & was cutting away from his body. As he finished the cut & as the knife released from the belt the knife slipped out of his right hand (still had hold of knife with left hand), moved upward & cut his little finger on right hand.
The injured employee was unbolting a 3 inch pipe flange. When it was completely unbolted the pipe sprang out of alignment, smashing his right index finger between the flange and another pipe, creating a laceration requiring sutures.
Mechanics were trying to break impeller from shaft. Employee had a bar wedged in the impeller and when pressure was applied to the shaft, the bar popped loose, and punctured employee's hand.
Employee was helping shift mechanic put bumper on 11 CM; mechanic hit keeper pin with a 4 lb. hammer. This caused a piece of metal to come off pin head and fly into employee's neck.
Employee was pulling on come-along handle to lift reducer when he felt pop & small amount of pain in right shoulder.
Report of Injury not received until 6/28/06. Employee was pouring Drano in clogged sink in lunch room. There was about 3 gallons of standing water in sink. Waited 5-10 minutes & then used plunger to try and free clog. Liquid in sink splashed him in face & eyes. Flushed eyes for approx. 30 minutes. Medcor transported employee to hospital.
Mechanics were lowering a tail pulley through an access port in the floor. When the pulley was tilted one of the cockers slipped, twisted, unlatched the safety clip releasing the chocker. This caused the mechanic's shoulder to be jerked down and to the right. So far only restricted duty has been prescribed.
Employee was using a 3" folding knife to strip insulation from a 500MCM power cable. As he was stripping the outer jacket away the knife slipped and he cut his lower left leg. He required sutures.
Mechanic was removing a flanged "Y" about 1-A Leach Sump Pump. When he pulled the last bolt out the pipe sprung six inches in the vertical direction smashing his finger in between the two pipe flanges.
Employee was separating a conveyor link on a crusher feeder using a steel wedge and a 4 lb hammer. On the second hit the wedge shot upward striking the employee in the upper lip and the two upper front teeth. Both front teeth required root canals. It is unknown if further treatment will be required.
Employee was standing on grating step attempting to align shear bolt holes on a 9" dust screw. On third pull of pipe wrench (using excessive force) wrench slipped off the screw section and his body fell back landing on a concrete pad. Struck his left elbow and forehead on concrete.
Using Bar Tool to remove impeller; felt pain in right shoulder. Incident was not recordable at this time because he wasn't on restricted duty and didn't miss any work at this time. The first day missed from work was 5/2/05 when he had surgery on shoulder.
Operator turning a valve with a pipe wrench heard a pop and felt pain in his left shoulder. Medical treatment was provided on 2/8/05 making this reportable as of that date.
Employee was struck on the hard hat by a bar. Employee received a laceration to his scalp requiring sutures.
Employee had ben working on a mantrip and was putting away the tools. The employee had his left hand full of tools and picked up the puller with his right hand. The puller slipped out of his right hand and fell striking the end of the middle finger on the left hand causing a laceration that required sutures.
While attempting to open bank 1 filter feed valve operator lost his balance when the feed valve gave. This caused him to loose his grip on the wrench and fall. The wrench then came off the valve striking him in the left hand. This contact by the wrench caused a fracture to the employee's left hand
Mechanic was pulling on pry bar to align pipe union when bar slipped and pinched the ring finger of his left hand between the bar and the receiver tank.
EE WAS ATTEMPTING TO LOOSEN THE PITMAN ARM FROM THE STEERING BOX ON A SUBURBAN. HE HELD ONE HAMMER STATIONARY AGAINST THE PITMAN ARM, WHILE HITTING IT WITH ANOTHER HAMMER. AS THE HAMMER MADE C ONTACT, IT SLIPPED, TWISTING IN HIS HAND & GLANCING OFF HIS OTHER HAND. THIS PEELED THE SKIN BACK BETWEEN THE THUMB & INDEX FINGER REQUIRING 14 STITCHES. HIS RIGHT HAND & THAT HAMMER HANDLE W
PERSON WAS PULLING ON WRENCH WHEN IT SLIPPED OFF BOLT AND STRUCK HIM IN THE MOUTH.
THE OPERATOR WAS CHIPPING OUT A HARD CHUNCK OF SODA ASH. HE WAS USING A HAMMER AND CHISEL TO DO THIS JOB. A GLANCING BLOW OFF AT THE CHISEL PINCHED HIS RIGHT INDEX FINGER BETWEEN CHISEL AND CHUTE BREAKING HIS FINGER.
EE WAS PULLING ON A PIPE WRENCH AND FELT PAIN IN BACK. THIS EE HAD RECENT BACK SURGERY. HE WAS SEEN IN COMPANY DISPENSARY ON THE DATE OF THE INCIDENT. HE LATER HAD AN MRI AND WAS GIVEN PHYSICA L THERAPY AND MISSED WORK FOR THIS TREATMENT. FMC BECAME AWARE OF THIS TREATMENT AND DAYS AWAY FROM WORK 3/6/03. THIS HAS BEEN DIAGNOSED AS A CTD & IS NOT CAUSED BY AN ACUTE EVENT.
EE & ANOTHER OPERATOR WAS TRYING TO BREAK A CHUNK OF TONA ORE ON THE BELT UNDER CP 2110 STOCKPILE RECLAIM FEEDER. EE WAS HOLDING A BAR PLACED AGAINST THE CHUNK OF ORE WHILE THE OPERATOR ATTEMP TED TO BREAK THE CHUNK BY HITTING THE OPPOSITE END OF THE BAR WITH A SLEDGE HAMMER. THE HAMMER GLANCED OFF OF BAR HITTING EE ON RT SHOULDER LD TIL SURGERY ON 8-30-02.
EMPLOYEE WAS HELPING MINER OPERATOR SPOT BITS WHEN BIT WRENCH SLIPPED OVER EXTENDING WRIST. EMPLOYEE HAS HAD PREVIOUS PROBLEMS WITH A WRIST. THE DOCTOR DECIDED THE REQUIRED FUSION WAS A RESULT OF CUMMULATIVE TRAUMA.
EE WAS USING A 12 LB SLEDGE HAMMER TO STRAIGHTEN TONGUE ON A TRAILER AS THE EE WAS STRIKING TONGUE WRIST BECAME SORE. AT THIS TIME IT IS BELIEVED BY DR THAT EE MAY HAVE DISLOCATED WRIST TEMPOR AILY AND MAY HAVE TENEDONE DAMAMGE.
DEGREE 5 ATTEMTING TO CONNECT PUSH RAM TO HEAD DRIVE W/ 3/8" TINSEL CHAIN. WHILE THE 3 EE WERE DISCUSSING HOW THE PUSH RAM COULD BE LIFTED HIGHER, THE CHAIN BROKE HITTING EE IN THE MOUTH AND J AW. REQUIRED STITCHES AND A FRACTURED JAW.
DEGREE 6. CUTTING DRIVE BELT OFF OF THE DRIVEN SHEACE WITH KNIFE (LOCKING BLADE) WHEN THE KNIFE BLADE WENT THROUGH THE BELT IT STABBED INTO HIS RIGHT LEG. I WAS NOTIFIED LATER THAT EMPLOYEE HA D RECEIVED STITCHES.
DEGREE 6. THIS REPORT IS LATE DUE TO MY OVERSIGHT. OPENING DRAIN VALVE ON T-16 TO DRAIN AND CLEAN TANK. USING A PIPE WRENCH AND HAMMER WHILE WASHING THE OUTSIDE OF THE VALVE. WHILE SWINGING TH E HAMMER, GLANCED OFF OF OVERFLOW PIPE AND STRUCK HIS HAND. RECEIVED STITCHES.
SCREEN AND ASSOCIATED EQUIPMENT HAD PLUGGED WITH WET PRODUCT. OPERATOR SHUT SYSTEM DOWN AND PROCEEDED TO CLEAN OUT SHUT. HE PODDED OUT SHUT AND USED A SLEDGE HAMMER ON THE OUTSIDE OF CHUTE. AT ONE POINT HE FELT A PAIN IN STOMACH. PAIN PASSED QUIKLY AND HE KEPT WORKING. OPERATOR NOTICED A LUMP ON THE STOMACH THE NEXT DAY.
THE STOCKPILE FEEDER WAS PLUGGED AT THE THROAT WITH AGGLOMERATED TRONA. EMPLOYEE ATTEMPTED TO DISLODGE THE CHUNK OF TRONE WITH A BAR AND HAMMER.AS THE CHUNK OF TRONA FELL IT HIT THE BAR HE WAS HOLDING ONTO. THE BAR MOVED SIDEWAYS HITTING HIM IN THE NOSE.
BOLTING DRISCO PIPE TOGETHER, THUMB WAS PINCHED BETWEEN THE BOX END WRENCH AND THE DRISCO PIPE. REQUIRED 3 STITCHES. DEGREE 6.
ATTEMPTING TO REMOVE TAIL SHAFT ON MINER TAIL. HOLDING PUNCH HAMMER, WHILE OTHER EMPLOYEE WAS HITTING PUNCH HAMMER WITH 4# HAMMER. BRASS HAMMER HIT PUNCH HAMMER GLANCING BLOW, CAUSING PUNCH HA MMER TO COME BACK AND HIT EMPLOYEE IN UPPER LIP.
PUTTING NAILS INTO THE END OF THE BELT ROLL. HE HIT HIS LEFT THUMB WITH A 40# HAMMER. CAUSED A LACERATION REQUIRING 5 STITCHES. DEGREE 6
EMPLOYEE WAS TERMINATED FOR VIOLATION OF COMPANY SUBSTANCE ABUSE POLICY; EMPLOYEE WAS UNDER SUSPENSION AWAITING TRANSPORTATION TO HOSPITAL FOR SELF-INFLICTED LACERATION TO LEFT WRIST/STITCHES.
WHILE TIGHTENING BOLTS ON CS-37 SPOOL PULLED MUSCLE IN LOWER BACK. THE DATE OF HIS INJURY HE JUST SAID HE FELT PAIN. ON 4/25/00 IT WAS BROUGHT TO OUT KNOWLEDGE HE WAS GIVEN MEDS AND SENT HOME FOR HIS INJURY BECAME BORE SEVERE. AT THIS TIME THE INJURY BECAME A DEGREE 3.
ASSISTING SHIFT MECHANIC REPLACE GASKETSON THE AUTOMATIC VALVE ON THE AGGLOMERATION LINE FOR THE CALCINER. TIGHTENING THE BOTTOM REAR BOLT WHEN THE WRENCH SLIPPED OFF THE NUT AND STRUCK HIM O N THE LEFT SIDE OF UPPER LIP. TRANSPORTED TO THE HOSPITAL RECEIVED THREE STITCHES, RELEASED FULL DUTY.
