EMPLOYEE WAS TIGHTENING BOLT ON THE STAY-NUT FOR ROLLS TAKE-UP ADJUSTMENT. HE WAS USING A BREAKER BAR WITH A SOCKET AND AN EXTENSION PIPE ON THE END. THE RELEASE PIN HOLDING THE SOCKET ONTO BA R WAS JAMMED SO WHEN FORCE WAS APPLIED BREAKER BAR SLIPPED OUT OF SOCKET RESULTING IN MAN\S FINGER BEING COMPRESSED BETWEEN PIPE AND GUARD ON FRONT OF LIFT-TRUCK ON WHICH HIS WORKING PLATFORM
