THE EMPLOYEE WAS WELDING ON THE TRACK ROLLER FRAME OF A D-9 CAT TRACTOR. HE WAS KNEELING ON ONE KNEE WELDING WHILE ANOTHER EMPLOYEE WAS USING A CHIPPING HAMMER ABOVE HIM. THE HAMMER SLIPPED AN D STRUCK FIRST EMPLOYEE HEAD.
