EMPLOYEE WAS USING A WEDGE ON A CHAIN TO DISLODGE A LARGE ROCK THAT WAS CAUGHT IN THE JAW CRUSHER. THE WEDGE AND CHAIN WAS JERKED BY THE WEIGHT OF THE ROCK AND EMPLOYEE STRAINED HIS LOWER BAC K WHILE HOLDING THE END OF THE CHAIN AT THE TIME OF THE JERK.

EMPLOYEE WAS TIGHTENING CRUSHER AND USING A SUSPENDED HAMMER RAM TO HIT THE LARGE WRENCH ON THE NUT WHICH TIGHTENS THE CRUSHER. EMPLOYEE WAS PUSHING THE WRENCH BACK SO THAT THE RAM COULD HIT FINGER AGAINST THE WRENCH.
