Employee was tightening a bolt on screen with a socket wrench. The Wrench slipped and slammed hand into the side of the screen. Employee seeked medical attention to ensure it was not broke.

Employee was releasing a chain binder from a load of crusher equipment. The binder twisted causing his right index finger to be crushed against the trailer frame and also resulting in a laceration that required sutures.
