Miner was cutting a belt using a hand-operated belt cutter. Miner applied force to the cutter which moved forward suddenly. Forearm was lacerated by cutter guide fence and/or securement device. Laceration closed by 12 stitches.

THE EE ATTEMPTED TO FREE UP A TROUGHING ROLLER WHILE THE PLANT WAS RUNNING. NO LOCK OUT TAG A WAS DONE. THE EE STRUCK THE ROLLER WITH A HAMMER AND WAS DROWN INTO THE ROLLER. A PUNCTURE WOUND WAS RECEIVED TO HIS LEFT FOREARM.

WHILE STRIKING A PUNCH OR CHISEL WITH A HAMMER A PIECE OF METAL BROKE OFF OF THE CHISEL AND PUNCTURED THE FOREARM IMBEDDING A SMALL PIECE OF METAL IN THE ARM.
