A 5-FOOT PRYBAR SLIPPED AS EMPLOYEE WAS OPENING SECTION, ALLEGEDLY CAUSING EMPLOYEE TO LOSE BALANCE AND FALL BACK WARD 2-FEET, WH9RE HE STRUCK A STATIONARY DRIVE MOTOR. EMPLOYEE DID NOT REPORT INCIDENT TO SUPERVISOR, DID NOT SEEK IMMEDIATE FIRST AID.
