Will pulling out a 30ft tape measure from tool box, the tip caught on tool box edge and when the tape exteneded out the miners right index finger rubbed against the side of tape cutting his finger.

Employee was working on the HP 400 cone crusher pounding out the pins for the bowl assembly and he was moving from one pin to the next walking with the 20lb sledge hammer when he felt his back pop and he pulled his lower back muscle.

Unbolting a head pulley wipe when the wipe came loose striking worker on the left pointer finger. The injury required a few stitches to seal.

EE was closing the lock on his box cutter style knife after changing the blade when his hand slipped from knife cutting the inside of his hand. Co-worker helped apply basic first aid to close cut and EE went to group health to receive 5 stitches. He was back at work by 10:30 am the same day.

Mechanic was using a torque multiplyer to remove a bolt from a yoke on a 800 komatsu loader. He went to change the direction switch on wrench and the handle came out of his grip rotating around and hitting him in the back of the head cutting him in a u shape, requiring 4 staples to close wound.

Individual was removing a 14" by 14" tool used to clean the middle deck of a 8 by 20 crushed screen, the tool slipped down cutting the right pinky finger through his glove. He was taken taken to local hospital where he recieved 12 stiches to finger.

Employee was standing approximately 4 feet off the ground on a ladder pulling towards him with a adjustable wrench; the wrench slipped off the nut causing employee to loose his grip with the ladder falling to the ground. He landed on his right side bruising his upper leg and scraping his head on a 4 inch rock when he landed.
