EMPLOYEE WAS USING A RAZOR KNIFE TO CUT STRINGS THAT ATTACHED TO A SPLICE IN A SECTION OF DETACORD. HE HELD DETACORD WITH HIS LEFT HAND AND AS HE CUT THE STRING THE BLADE ALSO CUT HIS LEFT IND EX FINGER.

EE WAS CLEANING LIME BUILDUP OFF THE SLAKER TRUCK WITH A SHOVEL. DURING THE PROCESS LIME GOT ON THE SKIN BELOW BOTH EYES CAUSING IRRITATION. FIRST AID WAS PROVIDED ON SITE. EE CONTINUED TO HAV E IRRITATION & WENT TO EYE DOCTOR. MEDICATION WAS PROVIDED & EE WAS TAKEN OFF WORK FOR ONE DAY.
