Cleaning out chute and helping move material from the hopper through chute. There is a broken vibrator on chute making it hard to move gypsum. Gypsum has a cement appearance. Left arm felt a sharp pain that went up to left shoulder.

Employee was removing bolts on the old GD gate. As he attempted to remove an old bolt with a wrench, it slipped off the bolt head, and his wrist was in-between the wrench and the gate.

Maintenance ee was using a hammer to re-shape an elevator bucket, when the hammer missed the bucket, glanced off the workbench and struck his index finger. Required 5 stitches.
