The morning of 1-3-26 @ 4:15am Employee was working on 92-11 in H.E. shop bay S1. The employee was in the process of removing a wiggins oil line with a 1 5/8 wrench, the wrench was on the fitting and they let loose of the wrench to reposition their body and the wrench fell off and chipped a tooth.

At the beginning of dayshift grinding operator reported an injury that occurred before getting off work on 9/6/25. The grinding operator was trying to unsand a sump pump pipe in 2004 by hammering on it with a mallet. When swinging at the pipe, the operator smashed EE's right ring finger on an angle iron fracturing it

A mechanic was removing a driveline from a vehicle. While prying the U-joint away from the yoke the pry bar slipped off the yoke and struck the employee in the mouth. This resulted in a chipped tooth and a lip contusion. Employee was seen by the medic and cleared to return to work.

A mechanic was removing an old G.E.T tooth from a 993 bucket using a wedge tool. Mechanic hit the wedge with a hammer, causing the tooth to fall landing on their right foot behind the steel toe of their boot.

Employee was changing out a hose when the 24mm wrench slipped, pinched and lacerated EE's left pinky finger. Employee was wearing chemical resistant latex gloves; not wearing cut or impact resistant gloves that are available. Employee was taken to our medical clinic and received 4 sutures to manage the laceration.

Two Mechanics were removing the Balderson from a 966M Loader. After driving out the connecting pin with a sledgehammer, the Balderson dropped 8" crushing the left foot of one of the Mechanics as the foot was unknowingly in the line of fire.

Millwright was removing a plug in an air compressor that was stuck at the time of the incident. When the plug broke free the Millwright's wrench slipped and crushed the right thumb between the wrench and the frame of the compressor.

A Warehouse employee was helping the Load Master, Pilot, & Co-Pilot push a pallet that was stuck. The pallet moved forward slightly then stopped. While the Warehouse employee was waiting to push again, the load master lowered the pallet on the pallet jack and pushed the arm/handle of the pallet jack forward, crushing the Warehouse employee's hand between the arm & the base

Employee had cut out a gussett out of 2" thick steel. The gussett would not come free when the employee tried to break it loose. The employee used two pry bars to get it free. In the process one of the pry bars slipped which caused the metal to fall and pinch the employees left hand middle finger.

Two workers were changing the ball joint on a 777 haul truck. A worker was reaching to neutralize the Duff Norton Jack when worker's hand entered the line of fire of the separation tool. The ball joint separation tool fell from its position under the barrel end of a 777 steer cylinder resulting in a crush injury. Worker received four sutures to right pinky finger.

Worker was utilizing a utility knife to trim a blackout window shade plastic guide rail. Using considerable force the employee did not maintain control and the knife cut through their glove and lacerated a finger. Employee was evaluated and treated by onsite medic resulting in sutures to the right index finger and returned to normal duties.

Electrician was using a Kline knife to remove the electric tape that is used to protect the electrical connections in the junction box on a motor. Using the knife in the left hand, pulled the knife towards self. The knife slipped and cut the right palm. Employee was wearing Hyflex 11-600 glove that are not cut resistant.

Driller Helper was using folding blade lockable knife to cut a slit opening into a tent. Pressure on the knife lock release from the material being cut allowed the knife to close and caused a cut on the Right index finger. WHILE CHANGING A FLAT TIRES ON FREIGHT TRAILER #74-303 THE JACK WAS HANGING UP AND NOT GOING UNDER THE AXLE AS EE PUSH AND TWISTED TRYING TO FORCE THE JACK UNDER THE AXLE HE FELT PAIN IN HIS RIGHT WRIST, HE GOT THE JACK UNDER THE AXLE AND FINISH CHANGING OUT THE TIRE. HE DID NOT THINK HE HAD HURT HIMSELF UNTIL LATER THAT NIGHT. AND REPORTED IT TO HIS SUPERVISOR THE NEXT MORNING.

While working on changing out the stop button, the employee was holding the cover with a pair of channel locks, while the other employee was cutting it with the sawzall. Employee was carrying some rented chain falls & come-alongs into the Mill tool crib. One of the handles from a come-along ratcheted down and struck him on the inside of his ankle, causing a small laceration. He had broken this ankle at home in Janurary 2009. He had major surgery to repair it, the laceration was in a location where screws were used to secure the bone.
