The morning of 1-3-26 @ 4:15am Employee was working on 92-11 in H.E. shop bay S1. The employee was in the process of removing a wiggins oil line with a 1 5/8 wrench, the wrench was on the fitting and they let loose of the wrench to reposition their body and the wrench fell off and chipped a tooth.

At the beginning of dayshift grinding operator reported an injury that occurred before getting off work on 9/6/25. The grinding operator was trying to unsand a sump pump pipe in 2004 by hammering on it with a mallet. When swinging at the pipe, the operator smashed EE's right ring finger on an angle iron fracturing it

A mechanic was removing a driveline from a vehicle. While prying the U-joint away from the yoke the pry bar slipped off the yoke and struck the employee in the mouth. This resulted in a chipped tooth and a lip contusion. Employee was seen by the medic and cleared to return to work.

A mechanic was removing an old G.E.T tooth from a 993 bucket using a wedge tool. Mechanic hit the wedge with a hammer, causing the tooth to fall landing on their right foot behind the steel toe of their boot.

Employee was changing out a hose when the 24mm wrench slipped, pinched and lacerated EE's left pinky finger. Employee was wearing chemical resistant latex gloves; not wearing cut or impact resistant gloves that are available. Employee was taken to our medical clinic and received 4 sutures to manage the laceration.

A Mill Mechanic was using a prybar to square a gland follower. The prybar slipped and crushed the little finger between the prybar and the pump case.

Two Mechanics were removing the Balderson from a 966M Loader. After driving out the connecting pin with a sledgehammer, the Balderson dropped 8" crushing the left foot of one of the Mechanics as the foot was unknowingly in the line of fire.

Mechanic was removing an o-ring from a coolant hose utilizing a metal pick tool. The pick slipped and punctured (approx. 1 inch deep) into mechanic's Left hand, in the web space between the thumb and index finger.

Mechanic was installing a drive axle tire assembly. When the tire bar EE was using to position the tire pushed thru the ice build-up on the concrete tire pad, EE felt a pop in EE's right shoulder

Employee sustained a small laceration/puncture on left forearm from a multi-tool while trying to change out a trauma strap. Sutures required to close the wound.

Employee's left hand was pinched between pump stuffing box and hammer handle. Resulting in an injury to left hand fingers.

Millwright was removing a plug in an air compressor that was stuck at the time of the incident. When the plug broke free the Millwright's wrench slipped and crushed the right thumb between the wrench and the frame of the compressor.

Mechanic was loosing an axle u-bolt using a wrench and breaker bar. While pulling wrench they felt a pop in their arm.

A Warehouse employee was helping the Load Master, Pilot, & Co-Pilot push a pallet that was stuck. The pallet moved forward slightly then stopped. While the Warehouse employee was waiting to push again, the load master lowered the pallet on the pallet jack and pushed the arm/handle of the pallet jack forward, crushing the Warehouse employee's hand between the arm & the base

Employee had cut out a gussett out of 2" thick steel. The gussett would not come free when the employee tried to break it loose. The employee used two pry bars to get it free. In the process one of the pry bars slipped which caused the metal to fall and pinch the employees left hand middle finger.

Employee was snipping metal banding off of a palletized item at the warehouse. When the metal banding was snipped the banding sprang toward the employee striking the arm area causing a laceration that required two sutures from the onsite medical staff.

Employee struck EE's left hand with a hammer while tightening the jam nut on the jaw crusher lower stationary side liner bolt.

Mechanic injured hand when pinched between handle of jack and truck body while placing jack to perform work. Mechanic continued working without issue for remainder of 4 week rotation. While on R&R mechanic went to doctor and was not able to return site for scheduled rotation on 12/03/2020.

Worker was cutting a piece of carpet when the utility knife slipped, resulting in a finger laceration. Required 4 sutures to left index finger to close the wound.

Worker was tightening bolts with a hammer wrench. Hammer slipped, and struck EE's left middle finger. Fracture to left middle finger.

Two workers were changing the ball joint on a 777 haul truck. A worker was reaching to neutralize the Duff Norton Jack when worker's hand entered the line of fire of the separation tool. The ball joint separation tool fell from its position under the barrel end of a 777 steer cylinder resulting in a crush injury. Worker received four sutures to right pinky finger.

Worker was utilizing a utility knife to trim a blackout window shade plastic guide rail. Using considerable force the employee did not maintain control and the knife cut through their glove and lacerated a finger. Employee was evaluated and treated by onsite medic resulting in sutures to the right index finger and returned to normal duties.

Employee lacerated EE's left hand cutting open a super sack of concrete with a utility knife

Mechanic was installing track pin with the 5P2380 track tool unit when the ram adapter fractured into 3 pieces. The mechanic was contacted on the right side of the face resulting in lacerations and fracture to the jaw.

EE was tightening a 2" hydraulic fitting with a 36" wrench when ee felt a pop in left bicep. Off site medical evaluation identified on 09/10/2017 that the EE had a ruptured left bicep

Metal shard broke off while hammering striking employee in left leg.

Mine Technician was using pocket knife to remove a plastic insert from a piece of survey equipment. Knife slipped and employee sustained a laceration to left hand between thumb and index finger.

As the employee was attempting to install the upper right corner of the windshield the tool slipped out of the rubber seal which caused left hand to come in contact with the center bracket lacerating left index finger.

Electrician was using a Kline knife to remove the electric tape that is used to protect the electrical connections in the junction box on a motor. Using the knife in the left hand, pulled the knife towards self. The knife slipped and cut the right palm. Employee was wearing Hyflex 11-600 glove that are not cut resistant.

Driller Helper was using folding blade lockable knife to cut a slit opening into a tent. Pressure on the knife lock release from the material being cut allowed the knife to close and caused a cut on the Right index finger. Employee was taken to Medic and received "a few sutures" and was immediately released back to work without limitations.

Employee was working from a 10' ladder swinging a 20lb sledge hammer (standard handle) to install the bed pins on a 777 Haul Truck while installing the second of 2 pins he felt a pain in his lower back

Employee cut another employee on the right hand while cutting a cloth top insert to remove the top bar from the filter cloth. Employee was not aware of the other employee's hand being in the line of fire when he made the cut cutting away from himself.

EMPLOYEE WAS REBUILDING A MAIN BEARING HYDRAULIC JACK. HE WAS HOLDING THE JACK BODY WITH HIS RIGHT HAND AND HITTING THE CYLINDER WITH A HAMMER IN HIS LEFT HAND. THE CYLINDER UNEXPECTEDLY DROPPED. HIS RIGHT HAND MIDDLE FINGER WAS IN THE LINE OF FIRE CAUSING THE TIP OF THE FINGER TO BE PINCHED BETWEEN THE JACK BODY AND CYLINDER.

Using a hammer pounding off scale on a lime tank, the hammer glanced to one side causing right hand to brush up against rusty sharp metal. Causing an cut to middle finger.

Employee cut left thumb with a box cutter. Employee was holding box with left hand while opening it with a box cutter on her right hand. The box slipped and the knife made contact with her left thumb requiring stitches.

The employee was working the Progressive Cavity pump slowly with a pipe wrench to clear fudged product. During this process the pipe wrench slipped catching his finger between the handle and floor plate.

Heavy Mechanic employee was removing the rod end pin from a production drill utilizing a manufactured drift from Caterpillar. The employee became fatigued from multiple strikes with 5# hammer which resulted in a mis-strike, hitting his hand.

Maintenance personal was removing a wheel seal from a trailer hub with a punch and hammer as he was hammering out the seal the hammer contacted the drum changing the hammer trajectory causing him to hit his thumb of his left hand that he was hold the punch with.

Mechanic was cutting a relay electrical connector in half with a personal pocket knife (in a tractor, behind the dashboard) the knife went through the connector into his left index finger cutting a portion of his skin off of the finger.

Employee caught left small finger with screw driver while trying to remove an internal lock ring. The lock ring was being removed to get access to the hydraulic tank to insert a oil sample tube.

WHILE CHANGING A FLAT TIRES ON FREIGHT TRAILER #74-303 THE JACK WAS HANGING UP AND NOT GOING UNDER THE AXLE AS EE PUSH AND TWISTED TRYING TO FORCE THE JACK UNDER THE AXLE HE FELT PAIN IN HIS RIGHT WRIST, HE GOT THE JACK UNDER THE AXLE AND FINISH CHANGING OUT THE TIRE. HE DID NOT THINK HE HAD HURT HIMSELF UNTIL LATER THAT NIGHT. AND REPORTED IT TO HIS SUPERVISOR THE NEXT MORNING.

Employee was moving a load with a pallet jack by pulling it with one hand when the pallet contacted a stationary object and came to a sudden stop pulling on the employees right shoulder.

While working on changing out the stop button, the employee was holding the cover with a pair of channel locks, while the other employee was cutting it with the sawzall. The blade slipped from the cutting surface and contacted the left index finger. This resulting in the laceration of the left index finger.

Employee cut hand with a pocket knife while preparing hoses to cover the threads on the adjustment bolts for a motor. Hose was not secured while cutting and use of improper cutting technique attributed to injury.

Employee was emptying a supersack of lime into a hopper. Employee was cutting the bottom of the bag when lime enter into right eye. Employee was wearing goggles over glasses, however the goggles were not properly fit and this caused a gap at the temples of the glasses.

Employee was stripping outter insulation from electrical cable. Employee was not wearing cut resistance gloves and was using improper cutting techniques. Employee lost control of the utility knife striking his left index finger, resulting in three stitches.

While stripping insulation from armoured electrical cable, knifed slipped cutting left thumb requiring stitches.

Employee was carrying some rented chain falls & come-alongs into the Mill tool crib. One of the handles from a come-along ratcheted down and struck him on the inside of his ankle, causing a small laceration. He had broken this ankle at home in Janurary 2009. He had major surgery to repair it, the laceration was in a location where screws were used to secure the bone.

EE was cutting a liner with a hooked blade knife. When cutting around a corner, the blade sliced through the top of his left hand, producing a 2" laceration that required stitches.
