Showing all 17EE was removing a thermocouple from the tower using a pipe wrench. The pipe wrench slipped off the thermocouple and pinched left pinky between the pipe wrench and the handrail.
While striking a bar with a hammer a small piece of the bar splintered off of it going into ee left forearm
Employee was removing a come along from a forklift and set of cribbing. Forklift shifted and employee's right middle finger got lacerated.
EE was lowering a blastline into a column when his left 4th and 5th fingers became caught between the pulley and the rope. This resulted in lacerations to these fingers. The employee's wounds were immediately treated on site and he was then taken to a clinic to be examined. He was diagnosed with a nail avulsion, given 3 stitches, and a tetanus shot. He was then released to work.
The EE was putting the pin gate in on 41WF06 with a hammer. He was in an improper position for the task and hurt his back.
EE was swinging a hammer at a return roller on a belt conveyor and missed it. This caused his hand to in between the roller and conveyor belt.
EE was prying on a door and the pry bar slipped. When the bar slipped it pinched his left index finger in between the pry bar and the kiln hood causing a cut to the tip of his finger.
EE was driving a pin into the drive track of the drill. After striking the pin with a hammer a small sliver of metal from the pin entered EE's left hand.
Employee was cutting skirt rubber with a utility knife. As he was cutting the rubber, he cut through the rubber faster than he anticipated and the knife slipped and cut his lower right thigh just above the knee.
Employee was pulling on a coupling with a porta-power and was hitting the coupling with a sledge hammer to jar it loose when a piece of the coupling broke loose and struck the employee in the face. The employee suffered some contusions on the left side of the face.
Employee was assisting with the retorque of a 1 1/2" Cap screw and re-agrivated and old hernial injury.
Employee strained the lower back using a 60 lb bar to strike the door to #3 ball mill to break the seal so the door could be opened.
EE was attempting to loosen a bolt for the drive mechanism on 34BC08 and strained his right shoulder while pulling on a wrench.
EE reports that in February or March of 2006, he strained his neck and L shoulder while dislodging oversized material at the Tower A grizzley. EE reported the incident to management on July 27, 2006. the incident is being investigated for compensability.
EE WAS SPLICING A CONVEYOR BELT, CUTTING THE BELT WITH A RAZOR KNIFE. HE WAS CUTTING TOWARD HIMSELF. THE KNIFE SLIPPED OUT OF THE BELT & WAS PULLED ACROSS RIGHT KNEE CAUSING A LACERATION ABOVE RIGHT KNEE CAP.
