EE was trying to open a food package with a knife. The knife went through the package and cut the EE's left index finger. The EE was taken to the hospital where EE received 4 stitches and returned back to work full duty.

Showing 50 of 66While torqueing hold down bolts for rear leaf springs on fuel truck, mechanic felt strain in left shoulder. EE was evaluated by MD and given restrictions on 1-22-16.
EE felt twinge in left shoulder while loosening fill plug on the rear circle drive of GR20. EE used 3/8 ratchet with no excessive force when shoulder made a pop. EE did not want to be seen by EMT. Eval 08/26/15 lifting restrictions of no > than 10 lbs. Employee had been placed back to normal role prior to 08/26/15 evaluation.
EE was preparing to run a test in the furnace. He had loaded the retort and was using a 3/4 inch combination wrench to tighten the bolts on the retort lid. While attempting to tighten one of the bolts, the wrench slipped and the wrench hit him in the mouth resulting in a chipped tooth. He saw his Dentist on 2-11-15 for the repair.
The material the employee was moiling broke free, causing the moiler to twist suddenly and strike his right middle finger. Employee was evaluated by a physician. No fractures were found. Employee was released back to work with a restriction of no moiling.
While striking a roller bearing with a ball-peen hammer, a small piece of roller bearing broke off and struck the employee in the right groin area. He was evalauted by a physician and taken off of work.
On 1/11/14 the employee was using a pry bar to dislodge jammed rocks in a drop chute, when the bar shifted and the employee's left ring finger was struck against the side of a metal hatch. He was evaluated by a physician on 1/11/14 and diagnosed with a fractured finger tip. He was issued a finger splint and released back to work without restrictions.
Employee was barring lodged materials when the bar struck his hand and injured his right ring finger. He was evaluated by a physician and diagnosed with a fractured finger. Sutures were administered and employee was released back to work with restrictions. Employee could not return to light duty work as scheduled due to taking narcotic pain medications.
While striking a lip shroud with a sledge hammer, a piece of metal broke off and struck employee's left calf. Employee was evaluated by a physician; sutures were administered and employee was released back to work with restrictions. Employee was scheduled off from 1/31-2/3 and was released back to full duty work as of 2/4.
The employee was using a 1/2 inch tap driven by a socket and ratchet combination to thread retaining blocks. While tapping, the tap broke and the broken end cut his left thumb while wearing gloves, causing a laceration. He was evaluated by a Physician and the wound was closed with sutures.
Employee threw a chain over a beam with his right arm and the needle nose pliers that were clipped to his jacket came up and him hit in the front left tooth and chipped it. He was evaluated by a Dentist and it was temporarily repaired on 11-9-11.
2 employees were in process of removing a pinion (bull gear). In order to get the pinion "pop" an employee struck the pinion with a hammer. The impact of the hammer caused a small chip to come off and strike another employee in the side. The employee was evaluated and a piece of metal was removed-resulting in sutures.
Employee cut right index finger while setting up to cut with Flexco belt cutter. He was evaluated by a Physician and sutures closed the wound.
Employee was welding on a filter vacuum elbow. He then raised his welding hood and was chipping slag. While doing this, a piece of debris flew back and got behind his safety glasses into his eye. He was evaluated by a Physician on 3-22-11 and given prescription eye drops for an abrasion.
Employee pinched his finger between wrench and support angle receiving a laceration to right pinky finger. He was evaluated by a Physician on 3-2-11 and sutures closed the wound.
Employee was using a hoist to disconnect a vibrating screen from a trailer, resulting in pain to his right wrist. Was evaluated by a Physician and on 4-7-09 had surgery resulting in lost time.

While attempting to free a stuck scraper in a chute, the scraper let loose hitting him in the mouth. Dental restoration was done on 9/19/2007.
Employee was removing a make up water valve. He struck his left thumb with a ball pean hammer - pinching his thumb between the hammer and the vertical piping for the washdown trays. He was evaluated by a physician - had an open fracture to his left thumb.
The rock spreader had chunks blocking the feed chute. The employee was barring to unplug the chute when a chunk fell on the bar and caught his hand between the bar and the feed chute. He suffered a chipped bone in his hand.
Employee was cutting rubber skirting with a utility knife. The knife slipped and cut him on the leg. He received sutures.

While trying to bar liner into place the weight of the liner shifted onto the pry bar. Employees back was strained while trying to accomodate the shift of weight. He lost one day of work.
Employee was taking the mechanism off the old screen from line 17. After cutting the last huck bolt off, he took a drift pin to punch the bolt out. When he did this the mechanism dropped down & drove the drift pin against the screen pinching his hand between the screen and the drift pin. This resulted in 3 stitches.
Removing trommel liners with porta power the liner was pryed from the trommel frame. It pinched employee's finger between the frame and the liner. He required stitches to his finger.
WHILE USING A PRY BAR TO REMOVE TROUGHING ROLLERS IT SLIPPED & STRUCK EMPLOYEE IN THE MOUTH. IT LOOSENED HIS FRONT TOOTH AND LATER REQUIRED A ROOT CANAL TO THAT TOOTH. MEDICAL WAS RECEIVED AND DETERMINED TO BE WORK RELATED.
WHILE USING A SLEDGEHAMMER TO DRIVE A PIN IN, A SHARD BROKE OFF AND STRUCK EE IN THE LEFT EAR. ON 12/09/03 IT WAS IDENTIFIED AND REMOVED. ONE STITCH WAS USED TO HOLD THE SITE IN APPROXIMATIO N.
WHILE CUTTING OFF SEALS AND TAGS ON FIRE EXTINGUISHERS HE REACHED FOR THE KNIFE ON CABINET, WENTTO CATCH IT AS IT FELL OFF CABINET, CUT RIGHT THUMB, WENT TO E.R. AND RECEIVED 2 STITCHES.
WHILE REMOVING A LINER BOLT, EE WAS HAMMERING WHEN HE MISSED THE PUNCH STRIKING HIMSELF ON THE RIGHT HAND, ABOVE HIS THUMB WITH THE HAMMER. MEDICAL TREATMENT SHOWED A HAIR LINE FRACTURE AND TH E HAND WAS CASTED.
STRAINED RIGHT SHOULDER WHEN HE WAS PULLING ON A PIPE WRENCH TO FREE UP A SPRAY NOZZLE. MEDICAL TREATMENT RECCOMMENDED PHYSICAL THERAPY TO START 3/4/03.
AS EE WAS MOILING ON A CHUNK IN THE UNIT 4 CODER THE MOILING POINT GOT STUCK IN THE CHUNK. THE CHUNK THEN FELL INTO THE HOPPER. EE TRIED TO HANG ONTO THE MOILING GUN TO STOP IT FROM FALLING IN TO THE FEEDER. EE ALLEGES THAT HE FELT A SHARP PAIN IN RT ANKLE. HE SOUGHT TREATMENT ON 12/20/02 & ON 12/21/02 HE WAS TAKEN OFF WORK UNTIL REASSESSED ON 12/23/02.
WHILE USING A SCREWDRIVER TO OPEN A CAN. THE SCREWDRIVER SLIPPED AND PUNCTURED HIS RIGHT HAND. HE REXCEIVED ONE STITCH.
EMPLOYEE WAS HOLDING A CASTING WITH A STEEL BAR. WHEN THE BAR WAS STRUCK IT SENT A SHOCK UP INTO HIS RIGHT SHOULDER. HE SOUGHT MEDICAL CARE 3-03. MRI DOES NOT REVEAL A TEAR. ANTI-INFLAMATORIES & PHYSICAL THERAPY ORDERED FOR 5/7/03.
EE'S RIGHT THUMB SWELLED AFTER PAINTING . IT LATER DEVELOPED INTO INABILITY TO EXTEND THUMB. WAS SEEN BY MEDICAL DR. & DIAGNOSED WITH A FRAYED EXTENSOR LIGAMENT REQUIRING SURGICAL INTERVENTION ON 07/29/02.
WHILE PULLING ON WRENCHES (WORKING ON A COBBER FEED PUMP) EE FELT A PULL ON HIS FOREARM AND SUBSEQUENT PAIN. HE HAD A LUMP DEVELOPE ON HIS UPPER RIGHT FOREARM. THE FOLLOWING DAY IT WAS RED & SWOLLEN. HE WAS SENT TO THE DOCTOR & PRESCRIBED PHYSICAL THERAPY. HE ALSO DEVELOPED CELLULITIS.
WHILE WORKING ON PUMPS & ADJUSTING BELTS, EE WAS PULLING ON A WRENCH WHEN HE FELT A PULL AND PAIN IN HIS RIGHT SHOULDER. HE HAD HAD SURGERY ON THIS SHOULDER IN THE PAST. HE WAS PUT ON LIGHT DU TY.
EE WAS REMOVING A BEARING STUCK ON THE TAPERLOCKBY STRIKING IT WITH A HAMMER. HE DIDN'T FEEL NORSEE A PIECE OF BEARING RACE BREAK OFF AND LODGE IN HIS LEFT FOREARM. HE REPORTED TO NURSE'S OFFI CE ON 9/14/01 DUE TO SWELLING. METAL PIECE REMOVED FROM FOREARM AND SUTURE PUT IN.
EE WAS BARRING BUILDUP FROM THE PREHEAT ON UNIT 1. HE WAS BARRING, LIFTING & PRYING TO REMOVE THE MATERIAL & AFTER A PERIOD OF TIME HE FELT STIFFNESS IN HIS BACK. HE WENT TO THE DR. ON 7-17 & WAS TAKEN OFF THE JOB.
EE WAS TIGHTENING LOOSE BELTS ON THE BALLING AREA CRANE RAILS. HE STOODED DOWN TO TIGHTEN THE BOLTS, BUT WHEN HE WENT TO STAND UP HE FELT A SNAP IN HIS LEFT KNEE. LATER IN THE MORNING HE WENT TO THE NURSE AND SHE SENT HIM IN THE CLINIC. HE WAS TAKEN OFF THE JOB.
EE WAS USING A PORTA POWER TO REMOVE A SLEAVE. THE PORTA POWER FELL OUT OF PLACE STRIANING HIS LEFT SHOULDER. ON 1-25-01 HE WENT TO THE DR BECAUSE HIS SHOULDER WAS STILL SORE AND HE WAS PRESCR IBED PHYSICAL THERAPY.
EE WAS INSTALLING LINERS IN THE OLD CRUSHER ROCKBOX THE CHAIN FALL EE WAS USING JAMMED UP & EE OVEREXERTED HIS SHOULDER WHILE PULLING ON CHAIN THE SHOULDER CONTD TO BOTHER EE & ON 1/4 HE WENT TO CLINIC & WAS RX THERAPY ON HIS SHOULDER
