VICTIM AND WITNESSES WERE TRYING TO REMOVE A BOOM PIN FROM A CAT 245 EXCAVATOR. THEY WERE DRIVING THE PIN OUT WITH A PIN HAMMER AND A SLEDGE HAMMER. AS THE SLEDGE HAMMER STRUCK THE PIN HAMMER A PIECE OF METAL SHEARED OFF THE PIN HAMMER AND STRUCK THE VICTIM IN THE CENTER OF HIS CHEST.
