EMPLOYEE BEAT ON LIME BIN WIH SLEDGE HAMMER TO LOOSEN LIME. HE NOTICED HIHS HSOULDER HURT BUT ASSUMED IT WAS MUSCLE PULL AND DID NOT IMMEDIATELY SEEK. MEDICAL ATTENTION. WHEN IT CONTINUED T O HURT, HE DID SEE DR. INJURY TO ROTATOR CUFF.

WAS USING A SLEDGE HAMMER TO LOOSEN A ROCK WHEN THE HAMMER SLIPPED & HIT THE CRUSHER BIN & THE HANDLE OF THE HAMMER GLANCED OFF BACK INTO HIS LEG, CAUSING A GASH THAT TOOK 26 STITCHES TO CLOSE .
