EMPLOYEE WAS REMOVING A FAN BLADE FROM A HAULAGE TRUCK. THE WRENCH HE WAS USING TO REMOVE THE BOLT HOLDING THE BALDE SLIPPED AND THE EMPLOYEE'S HAND STRUCK THE FAN BLADE. HE CUT HIS INDEX AND SECOND FINGER ON HIS LEFT HAND. THE FINGERS WERESUTURES.
