In an attempt to close a liquor feed valve with a valve wrench, the valve broke the process, resulting in the wrench slipping and smashing the employee's right pinky finger.
Employee notice product (soda ash) was not feeding adequately on to the 1410 belt. Employee began to rod out blockage. While rodding out though the gate on #2 Bin, the blockage broke free and hot soda ash overwhelmed employee trying to get out of the way of the product. Employee received burns to mid-thigh and calve areas on both legs.
Employee was cutting foam board with utility knife. While finishing cutting the board, knife slipped and caught employee's left ring finger. Employee received stitches in finger and returned to work.

While trying to align bolt hole with a bar on a take-up frame and ram frame on a belt drive, mine employee pinched right pinky finger between bar and ram frame. Mine employee broke tip of the top bone of the right pinky finger.
Employee was operating torque gun to tighten bolts on # 2 filter and pinched EE's left index finger to the shell of the filter. Left index finger was lacerated and required stitches.

Tightening canopy bolts on a shuttle car, miner slipped off bolt and felt a pop and burning sensation in EE's left arm and shoulder. After testing and evaluation from the doctor, the doctor's report on 7/22/2019 to mine that this will be a reportable injury. Miner will get surgery 7/29/2019.

Employee was using a sledge hammer to unplug chute when ee felt a pop in left shoulder. Employee went to doctor on 7/20/17 for MRI. On 8/2/17 it was determined that ee had a torn muscle and that it was work related caused by the incident. The employee is currently off work.
While using a pry bar assisting in adjusting a pinion on a dryer the bar slipped and pinched the tip of the right hand ring finger. This action caused a nondisplaced distal phalanx fracture

EE was trying to close #3 dryer overhead valve. The valve was very hard to operate. He hurt his right shoulder while struggling to close the valve. He was using a 36" pipe wrench as per usual.

Two employees were working together to clean up dust piles. EE was shoveling and other ee was using a pick. Other accidently stuck ee in the right forearm with the pick. The result in a puncture wound and sutures.

EE was removing bolts out of the drive end of the #6 dryer feed screw on GR3 EE was using a bar to keep screws from turning, when he hit the bolt the screw moved causing the drive pin to push finger up against screw housing. This act resulted in sutures to the 4th digit on the left hand and a fracture to the left wrist.

Employee was using a metal wedge and a ball pien hammer to obtain a proper gap between two pieces of pipe that were to be welded. When employee was hammering wedge into place with hammer, he hit his thumb and the hand holding the wedge. Fractured thumb on the left hand between the nail and the first knuckle.
