Employee was wire stripping using a Klever Kutter when the plastic edge of the tool broke causing the tool to slip and lacerate the palm of the employees left hand. Employee was taken to the Urgent Care and received 5 stitches with a return to Work as Tolerated.

Maintenance worker was changing out a broken mirror on a haul truck when the wrench slipped off a bolt causing the left index finger to come into contact with the mirror resulting in a laceration that required 4 sutures
EE pulled down on a Chain Spreader tool handle. The handle suddenly released downward straining employee's shoulder. Employee saw a physician on 9/9 and released back to work as tolerated. On days off the employee saw an Orthopedic Physician 9/30 and was released restricted. The employee returned to next scheduled shift restricted on 10/5 making this a reportable case at that time.
Operator was cutting a piece of flat cardboard with a pocket knife, the knife did not cut all the way through in an area. The operator then pushed harder with the knife causing it to slip and cut his left hand at the base of his thumb. The operator was transported to the ER room where he received (2)two sutures.
Operator was changing GET using deadblow hammer and punch when a piece of the GET flew off and penetrated operator's left leg above the knee. The operator is scheduled to have the metal removed on 7-27-2011. The operator has not missed any work to date.
Mechanic was removing defuser from hydraulic filter with hydraulic filter still in the housing. Pulled the defuser out with pliers and the whole filter came out, pinching the middle finger between filter and housing. Mechanic received five sutures in middle finger.
Employee was digging a trench with a spade. Impact from the hard ground while digging caused pain in left shoulder.
Employee was using a vise grip, holding a punch to pound out a u-joint on a pick up truck driveshaft. Hammer glanced off of punch and hit left index finger. X-ray revealed a fractured bone.
Employee was picking up the end of a tow strap when he felt a pull in his lower back followed by slight numbness in right leg. Doctor visit revealed a pulled muscle in lower back.
Employee was cutting a piece of plastic liner with an exacto knife when his left hand. slipped causing the knife to come away from the plastic. Momentum of the hand and knife continued on cutting his right forearm. Laceration to right forearm.
Employee was hitting a loader tooth with a sledgehammer to loosen it up for removal. Small piece of steel from tooth broke off and impaled itself in employees right leg. Steel removed and leg stitched up.
EE WAS SHOVELING MATERIAL WITH HELP FROM ANOTHER PERSON. SHOVEL TIP FROM OTHER PERSON'S SHOVEL STRUCK EE'S THUMB CAUSING A LACERATION REQUIRING 5 STITCHES.
EMPLOYEE WAS CUTTING A DRIPPER LINE WITH AN EXACTO KNIFE. LINE WAS PARTIALLY CUT AND BEGAN SPRAYING SOLUTION. EE ATTEMPTED TO FINISH CUTTING LINE WHEN HE CUT HIS LEFT INDEX FINGER. STITCHES REQUIRE TO CLOSE LACERATION.
EE WAS WORKING AT THE CRUSHER, ASSISTING VULCANIZER SPLICE ON CONVEYOR BELT. HE WAS CLAMPING TWO 2X4 PIECES OF LUMBER ONTO THE BELT. ONE OF THE BOARDS POPPED UP HITTING HIM IN THE MOUTH. LACER ATION TO LOWER LIP. ONE 1 STITCH REQUIRED.
EE WAS ATTEMPTING TO LINE UP A DRILL BIT SO A WRENCH WOULD FIT ON THE BIT. WRENCHED DROPPED DOWN ONTO BIT WHILE HIS HAND WAS STILL IN PLACE. RIGHT INDEX FINGER PINCHED. STITCHES REQUIRED FOR L ACERATION. NO FRACTURED BONES.
