Showing all 13Employee was using a pocket knife to cut a zip tie. When the employee cut through the zip tie, employee cut into left forearm.
Employee was replacing bolts and wrench slipped off causing employee's left thumb to contact the hammer base. DOI was 09/03/20, did not seek treatment on DOI. Emp was scheduled for vacation on 09/04 thru 09/13. Upon empl return of 09/14/20 from vacation, emp informed the employer that treatment was sought on 09/12/20 and the care facility confirmed a fractured left thumb.

The miner was cleaning out a chute with a pry bar when the bar broke through and he caught the bar pulling muscle in lower back
Employee was breaking clods with a sledge hammer when he injured his hand. He also suffered a lime burn to his wrist causing cellulitis
The employee was helping adjust the clutch on the Guzzler with a pair of channel locks when it slipped and a sharp piece of metal cut his finger and nail. Fingernail had to be removed.
Employee was cleaning out the preheater using a pry bar and complained the following day of testicular pain. He has restrictions that prevent him from working.
Employee was cleaning out transfer chute of #5 kiln when he was struck by a 10' piece of conduit he was using. NO LOST TIME OR WORK RESTRICTIONS, however he received 5 stitches at the hospital emergency room.
Employee was adjusting kiln trunnion, missed the knocker wrench and hit himself on the leg. No Lost Time, however he has some work restrictions and is receiving physical therapy.
EMPLOYEE WAS CLEANING SCRUBBERS. HE WAS USING A HAMMER AND HIT HIS ELBOW ON A BEAM. INCIDENT WAS NOT REPORTED UNTIL 11/12/01 AND EMPLOYEE DID NOT AST TO SEE A DOCTOR UNTIL 11/21/01. NO LOST TI ME. MEDICAL ONLY AS HE IS TO GO TO PHYSICAL THERAPY FOR 4 WEEKS.
EMPLOYEE WAS CUTTING A BELT. WHEN HE MOVED BACK TO STAND UP. HE SLIPPED ON A ROCK AND CUT HIMSELF REQUIRING 5 STITCHES. MEDICAL TREATMENT ONLY. NO LOST TIME.
