Employee was prying bearing housing off with pry bar on a rotary and was exerting a lot of force when pry bar slipped and they felt their right wrist pop and there was an immediate sharp pain.

When loosening bolt socket came off and thumb hit sidewall of housing between ratchet handle and the side wall. Thumb is sore and swelled some ice and Ibuprofen was used at home. Thumb is sore and has some limited range from stiffness and swelling but has a limited range of motion at this time.

Employee was repairing a broken wooden pallet with a hammer and nails. Employee missed the nail and struck their left thumb with the hammer, resulting in a cut, bruise, swelling, and hairline or "turf" fracture.

Utility operator strained right on 3/16/2018 using a sledge hammer and placing rail car vibrators. Operator initially denied medical treatment but did seek medical attention on 5/10/18 followed by acupuncture on 5/10/18. Treatment continues without restrictions

Two middle fingers were caught between derailer and railroad track when switching derailer to closed position. Employee sustained a cut to third finger which required two sutures and the fourth finger sustained a hairline fracture. In accordance to company policy, the employee underwent drug and alcohol testing which yielded a failed result.

Employee struck herself in the face with a hand tool while attempting to open the lid of a 5 gallon bucket. The bucket opening tool struck the employee in the cheek and required 2 sutures for cosmetic closure.

Employee reports that he strained his low back on 3/5/13 while using a sledge hammer to break ice/ore. The employee sought medical treatment and was diagnosed with nerve impingement on 4/5/13. The employee received a series of medical visits which included treatment beyond first aid.

EE LACERATED HIS LEFT MIDDLE FINGER WITH A SAFETY UTILITY KNIFE WHILE ATTEMPTING TO STRIP THE INSULATION FROM A SERIES OF RIGID 500 MCM WIRES DURING THE REPLACEMENT OF A MAIN DRIVE MOTOR FOR A N AIR CLASSIFYING MILL. THE LACERATIONS REQUIRED 6 SUTURES. THE EE RETURNED TO WORK SAME SHIFT WITH NO RESTRICTIONS.

WHILE USING A UTILITY KNIFE TO CUT THE PLASTIC TABS ON COVER TO A 5 GALLONG BUCKET THE UTILITY KNIFE CUT THROUGH THE PLASTIC EDGE OF THE COVER AND PROCEEDED INTO HIS RIGHT THIGH.

WHILE REMOVING PACKING SLIP FROM A PALLET OF TALC WRAPPED IN PLASTIC USING A UTILITY KNIFE, EMPLOYEE PULLED UPWARDS ON THE PACKING SLIP WITHRIGHT HAND, CONTACTING THE OPEN BLADE WITH THE PALM OF THE RIGHT HAND. EMPLOYEE SUSTAINED A SUTURABLE LACERATION TO PALM OF RIGHT HAND.
