EE was cutting sheath off wiring, the blade slipped and caught left middle finger. EE and another employee rendered first aid, clean and wrapped the cut. The cut did not require additional medical as it appeared to be healing fine. The Evening of 3-4-26 the finger swelled up and they went to the ER where It was determined it was infected and medical intervention was necessary.

The miner took Ibuprofen & went to EE's chiropractor that night. 9/14 while lifting ordinary grease gun pain increased & manager sent Miner to ER- Diagnosed with sprained shoulder. Prescribed Muscle relaxer and rest-to return to work 9/20
EE was driving wedge to lock cone crusher down with a sledge. when a piece of unknown debris bounced off safety glasses and was deflected into eye. when ee was seen at er-nothing was in his eye-it was washed out at plant. he had a small laceration.
EE WAS PRYING ANGLE IRON OFF FORD TRK & JAMMED HIS THUMB. AT THE TIME HE THOUGHT IT WAS JUST SPRAINED. IT CONTD TO BOTHER HIM AT WHICH TIME HE SAW A DR RECENTLY
