While tightening the nose cone bolt with his foot and holding the wrench with his left hand and keeping the bolt from turning with his left foot, the bolt turned catching his left index finger between the end of the wrench and nose cone. This smashed his left index finger.

EMPLOYEE WAS USING A 2 LB SLEDGE HAMMER AND A BOLT TRYING TO REMOVE ANOTHER BOLT OUT OF THE HAMMER MILL ROTOR. HE SWUNG THE HAMMER AND HIT HIS LEFT THUMB CAUSING MEDIAL AND LATERAL LACERATIONS AND CRUSHING THE TIP OF HIS LEFT THUMB.
