Employee was cutting a splice on shuttle car when box knife broke. Momentum caused knife to cut left pinky finger. Employee returned to work with restrictions.
Employee was cutting skirting for belt line. Knife cut through skirting cutting the employee's left thumb. Employee was evaluated and required stitches. The employee returned back to work.
Employee working to empty a bin hit their left middle finger with a rubber hammer. The employee was evaluated and was found their finger tip had been fractured. The finger was wrapped and padded and the employee returned to work.
Employee was putting drive belts on a screen deck. Was using a pipe wrench on the shaft to turn the counterweight. When the counterweight came over the top, it spun pulling the pipe wrench down and catching the little finger on the employees left hand pinching it. The employee was transported to the doctor and required stitches.
Operator was helping to get a piece of drill steel that had gotten stuck in the back loose. The operator was using a pipe wrench to grasp the steel and was attempting to pull it out when their thumb was pinched in between the wrench and bottom of top plate of bolter. The employee was taken to the hospital for evaluation and released back to work with restrictions.
Operator was removing bolts on Evaporator Body demister door and the bolt loosened up. The wrench slipped smashing/cutting the operator's little finger between the wrench and the floor grating. The operator was wearing gloves. The operator was transported to Sweetwater Memorial Hospital for observation, and it was determined it needed two stitches.
Employee was mopping the floor when she felt a pop in her thumb, she continued to mop. Then she felt a second pop in her thumb and reported it to her supervisor. Was advised to seek medical attention
Employee was holding a sheave wheel block during a repair, while another employee was using a punch hammer and a 4 lb. hammer to free wheel. Punch hammer glanced off of wheel and struck other employee's finger tip. Employee received medical attention, finger was fractured. Employee was able to return to work with restrictions.
Employee was removing a broken bit from continuous miner drum, using a punch hammer and a 4 pound hammer. A projected piece of metal sliced employee's forearm and was embedded. Employee was given medical attention, metal was removed and sutures given to repair laceration.
Employee was installing outlet gate under railcar when outlet gate shifted on jack and pinched fingers between outlet gate and jack. Resulted in fractured little finger.
A burner box was being hoisted away from duct work on kiln in Unit 6. As employee was pulling the chainfall, two eyebolts (connecting the rigging) failed, allowing the burner box to fall to the deck and push employee backward. Employee injured right knee and received laceration to left hand.
Employee was using a hammer to knock out broken bit from head of continuous miner. Metal chip was released and was imbedded in employee's forearm. Metal was removed by physician and employee received one suture. Employee returned to work with no restrictions.
Employee was attempting to remove a piece of rock from a screw conveyor. The pipe wrench he was using came into contact with the conveyor pinching his left hand.
Employee was installing a new chain on feeder breaker (SL05) using a Port-a-Power tool. The tool cylinder jumped out of place, hitting employee on the shin. One week later, employee received medical attention and injury had become infected, resulting in lost work days.
EE was fitting cone to burner using a come-a-long. The come-a-long was hooked to eye bolt. The eye bolt broke and the come-a-long hit EE in the face and mouth.
EE was driving posts for Mine Rescue training field, slipped and cut left index finger. Required 7 stitches.
EE WAS TRYING TO REMOVE A SIDE STAB SHOE BRACKET USING A HYDRAULIC PORT-A-POWER. EE FELT A PAIN IN HIS BACK WHEN HE TWISTED TO ACTIVATE THE POWER SWITCH.
EMPLOYEE WAS ATTEMPTING TO REMOVE A PIECE OF RUBBER HOSE AND DUCT TAPE FROM COLLISION CABLE. HE WAS USING A UTILITY KNIFE TO REMOVE THE HOSE AND THE KNIFE SLIPPED, CUTTING HIS LEFT INDEX FINGE R. SEVERAL SUTURES WERE REQUIRED.
EE WAS CHANGING A DRILL BIT ON THE BOLTER WHEN THE PIPE WRENCH SWUNG AROUND, STRIKING THE EE INT HE NOSE, WHICH REQUIRED 3 SUTURES.
WHILE USING A RATCHET TO REMOVE BOLTS, EMPLOYEE FELT A PAIN IN HIS SHOULDER.
WHILE CHANGING BITS EMPLOYEES HAMMER GLANCED OFF INSERT AND HIS FINGER WAS CAUGHT BETWEEN HAMMER AND BIT CAUSING A LACERATION REQUIRING SUTURES.
EE WAS PRYING A ROCK THAT WAS STUCK IN THE CONVEYOR CHAIN OF THE CONTINUOUS MINER WITH A PRY BAR, WHEN HE FELT SOME PAIN IN HIS RIGHT SHOULDER. THE DOCTOR GAVE HIM TIME OFF UNTIL THE 5TH OF DE CEMBER 2000. I WAS NOT NOTIFIED UNTIL 11/27/00 ABOUT THE ACCIDENT.
