Changing drill hammer on drill rig putting on Jay wrench onto the hammer and wrench dropped on employee's left hand crushing the tips of EE's two middle fingers. EE needed stitches to close the cuts on both fingers.

While cleaning out a rip-rap chute with pry bar a rock moved and the pry bar struck the employee on the right shoulder. Xray was negative. Slight bruise to right shoulder.

Ee was clearing diester of rock that was blocking passage. He was using a sledge hammer, the handle broke, causing right hand to strike rock.
