Driller needed old conveyor belt cut for dust covers. Driller helped cut out an example and instructed employee to cut two more out. While cutting belt with a utility knife with right hand knife slipped off of belt and struck left hand below the thumb.

while changing drill stem on blast drill, pipe wrench would not bite stem. EE was helping it bite the stem. When it did bite, it pinched EE's right hand between wrench and boom mast.

Employee's task for the day was to help change the wear plates on the secondary crusher. While unbolting the old wear plates it was cracked and the piece that was cracked came off and pinched his left pinky finger.

EE's job assignment was stripping old floor tiles off of subfloor in q.c. lab. He was using a floor stripping hand chisel. EE was on subfloor and had left arm in front of him for support, the chisel slipped out from under the tile and struck his lower left arm. This resulted in a laceration on lower left arm. No restrictions.

EMPLOYEE WAS CUTTING A PIECE OF CONVEYOR BELT, PULLING THE KNIFE TOWARDS HIMSELF WHEN THE KNIFE SLIPPED AND CUT EE'S LEFT LEG ABOVE THE KNEE RESULTING IN AN INJURY REQUIRING (3) STITCHES TO THE SURFACE OF THE WOUND.
