The EE was in the process of removing a conveyor belt. The EE was using a utility knife to cut a hole in the belt to place a chain through the belt to remove it. While the EE was cutting the belt, the blade came out of the utility knife and cut his finger.

Employee had completed taking off outer lid of a 5 gal bucket of oil. The employee pulled up on the bucket spout with his right hand when the pull tab on the spout broke causing his right hand holding a box knife to cut his inner left arm.
EMPLOYEE WAS CUTTING BELTING MATERIAL WITH A POCKET KNIFE WITH HIS RT.HAND WHEN THE KNIFE FOLDED AND LACERATED HIS RT. INDEX FINGER.
EE WAS CUTTING BELTING MATERIAL WHEN KNIFE SLIPPED AND STRUCK HIS RIGHT KNEE CAUSING A LACERATION.
EE WAS HOLDING ONE FITTING W/A WRENCH & ANOTHER EE WAS USING A WRENCH TO TIGHTEN BOLTS. WHEN THEOTHER EE PUSHED ON THE WRENCH IT SLIPPED HIT EE IN THE MOUTH CHIPPING 2 TEETH.
EE WAS STRIKING A METAL PUNCH BAR WITH A 3 LB. HAMMER, ATTEMPTING TO REMOVE ROCK THAT WAS HUNG IN CONVEYOR BELT PULLEY. A PIECE OF METAL BAR BROKE OFF AND STRUCK EE IN THE LT. FOREARM CAUSING A PUNCTURE WOUND. SMALL PIECE OF METAL HAD TO BE REMOVED FROM HIS ARM. JUST HAVE TO KEEP ARM DRY & CLEAN.
EE WAS ASSISTING IN REMOVING A GUARD WHICH HAD AZIPPER TIE THAT WAS HOLDING IT SECURE ON THE SIDE. WHILE CUTTING THE TIE, THE KNIFE SLIPPED AND CUT HIM ABOVE THE RIGHT KNEE.
EE WAS PULLING ON A PRY BAR TO WINDER A SLOT FOR LEG ON A CONVEYOR TO FIT IN. PRY BAR SLIPPED CAUSING EE TO FALL BACKWARDS TO THE GROUND ON LEFT SHOULDER.
EE WAS CUTTING A 2" WATER HOSE. EE'S INIFE SLIPPED ON THE WATER HOSE, CUTTING HIS LEFT INDEX FINGER.
