Showing all 7Employee was working with hand wrench and impact driver to loosen bolt. Lost grip on wrench open end whipped around caught EE on the side of forearm. Onsite medical treatment was provided, however, on 7/10/24 the employee notified us that EE thought the laceration was infected. We took EE to the Dr. and it was, so they gave EE some antibiotics.
Patching belt using a personal knife. cutting patch knife slipped and they cut their knee.
EE was cutting a piece of conveyor belting to put on a conveyor pan for a wear item at Evans Wash Plant.
Employee was tightening screen deck bolts. When the wrench slipped and hand hit another bolt to the side of the one they were tightening. It caught a finger and cut it open.

Repairing crusher - using a come-along, at maximum tension come-along slipped off at hook end flying back and striking miner right side of face. Laceration near rt eye, bone chips (fractures)of cheek bone, stitches 20, no surgery req'd, eye and vision OK.
