The IP went to work night shift 04/01/2026 while working at around 4:30 in the morning EE was straining with a pry bar to pull a replacement hydraulic hose into place, the IP felt a pop in EE's right shoulder. Then the IP called supervision and went to the clinic to be assessed, then sent to Fairbanks hospital, MRI determined surgery was needed.

Showing all 31The IP is employed as a mechanic and was leaning over vehicle trying to loosen seized bolt when had onset of lower abdominal pain.
Mobile mechanic had been torquing a fastener on a loader bucket tooth when the hand tooling slipped, resulting in the mechanic twisting the left knee. "The accident occurred on 8/29/25, however, the EE was cleared to go back to work the same day. The EE chose to return to place of residence and got an off-site diagnosis, which was provided to the mine operator on 9/23/25."
Diamond driller was removing a bit from the barrel using a pipe wrench. When the bit broke free, EE's right hand went past the pipe wrench and hit the rib.
Graduate Engineer was tasked with getting a guide tube. While opening a new bundle of guide tubes with a pair of Klein tools the band snapped back, causing a laceration between the 4th and 5th fingers of the right hand. The engineer was not wearing gloves at the time of the incident
IP was using a Parmelee wrench to break latch head by applying downward pressure when EE's hand slipped. The wrench spun around the tube. When the IP's hand slipped it caused EE's head to lower and the wrench struck EE in the lip, causing a small vertical split in EE's lip.
During annual firearms training the injured employee had shot several other guns when they used the .338 rifle. After the first shot they adjusted their stance placing the scope close to their face, upon shooting the kick back caused the scope to contact the bridge of their nose causing a laceration. It was repaired with 7 sutures.
First Aid- 4 stitches - Returned to full duty. While attaching a multi tool to a scaling bar, miners hand had slipped and made contact with the blade of the multi tool, causing approx 1" laceration to the thumb of EE's left hand.
The worker was using damaged broom that broke during the sweeping and broken metal handle caused cut on the workers hand/finger. After receiving 10 stitches worker was returned to regular duties.
The IP was repairing SSE 83 battery cables. As a piece of heat shrink was being cut to length, the tips of the cable cutter cut the left index finger.
While mixing Xanthate operator smashed finger between hammer handle and bottom of barrell of Xanthate.
Employee was changing out G.E.T and was informed to use a steel hammer to remove then a copper hammer to reinstall. EE struck G.E.T with the steel hammer and the third one EE hit had broken a shard of hardened steel and it lodged into EE's finger. EE went to the on-site medic and had it removed, then got three stitches.
Drill Helper was emptying a 10ft NQ inner tube. EE struck the inner tube with a dead blow in an attempt to free core and dislodged a small piece of core from the lifter case. The rock fell along the tube and struck the drill helper on the upper lip leaving a small laceration.

IE was moving a pallet of materials with pallet jack. IE was pushing loaded pallet jack with materials into place and strained left lower leg.
Employee was cutting asparagus with wet hands and no cut glove and cut EE's right index finger
A mechanic complained of irritation in right index finger. EE was taken to site medic a metal sliver was removed from finger and first aid dressing was applied. The mechanic noticed swelling in finger 3 days later, EE was taken to a doctor. The doctor diagnosed an infection and prescribed an antibiotic.
Employee states "after I had measured the depth of drill hole for casing set-up, I attempted to retract the tape." "It appeared to be stuck, yanked on the tape, and it cut through my glove." Employee sustained minor laceration (6 stitches) to webbing area between the ring and little fingers of Right hand.
The employee was descaling a 5' section of 4" HDPE pipe with a small sledge hammer, when he struck the pipe the pipe bounced up and struck him above the left cheek; resulting in a small laceration which received three sutures.
Employee was cleaning pre-dampener holes on shotcrete pot that were clogging with wet shotcrete . Employee used a spud wrench instead of a piece of wood which is routine. The auger caught the wrench and slammed his two fingers into the auger channel.Brusing one finger and cutting the tip of the other. Employee was wearing cut resistant gloves at the time.
EE was cleaning chuck jaws with a wire brush. With the motion EE was using he pulled material directly toward his face. EE didn't notice anything hit his glasses or his eyes, but later in his shift he felt an irritation. He did have eye protection on at the time of incident and reported this to his foreman at the end of his shift.
Miner was cutting shrink wrap off new MLC (Mine Load Center). He was cutting with his right hand and pulling the wrap with his left. While performing this task he lacerated his left thumb resulting in six sutures.
Employee was dislodging a wedged rock from between the rear tire and the cab of loader and felt a pop in several places in his right forearm. Strain to right forearm.
Driller assistant was placing an unsecured wrench to loosen a bit from a core barrel when the driller pulled the rotation lever. This caused the driller assistant's hand to be caught between the hand of the wrench and the mast of the drill rig.
Stacking liners on a pallet. One was sideways, tried to lift it on pallet using prybars. The liner rolled the opposite way causing the bar to release. Britt lost his balance and tried to catch himself injuring his left upper arm
Employee was working fro Teck-Cominco at the time and was beating on a pressurized paste pipe when the plug broke loose. The pipe with the pressure releasing swung over & knocked employee over on his right shoulder, breaking his collar bone.
While tightening a bolt on a 8" vic clamp on a paste pipe line, he was pulling on the wrench when he felt a pop in his right shoulder and it gave out and had alot of pain.
Attempting to remove a heater core from D9 Dozer. Ee trying to remove a bolt overtorqued the wrench slipped off of the head of the bolt. This caused ee to lose his balance. He was standing on the track of the dozer. Ee fell approx 4 ft to the ground causing pain in his right shoulder. Ee was returned to work the same day as the injury.
Employee was using a razor knife to cut holes in a piece of rubber belt. the razor knife unexpectedly came out of the material and struck the employee at the base of the left thumb, causing a laceraiton. He developed an infection on the 10-12-05 and received a perscription of antibiotics.
