Employees right-hand middle finger pinched between the drill pipe & lifting strap. Xray was negative for fracture/dislocation with minor bruising to finger. Medical provided performed fingernail trepanation & issued oral oxycodone & Ibuprofen. Second opinion requested -Stated that with conservative care such as O/C ibuprofen /ice packs would achieve the same desired outcome as FAI.

Showing all 27EE was attempting to put an insulator on an anode which was about 4" off the ground. As the EE went to hammer the insulator it caused the opposite side of the steer horn to come off the strongback. This resulted in the employee's hand being pinched between the strongback hook and the steer horn. EE was seen at ER and GHR clinic and was released back to work with restrictions.
EE was helping to replace hoist ropes on Shovel. EE was trying to pry the hoist ropes to the next rung on the hoist drum. EE lost contact with the rope causing pry bar to break free, smashing EE's left pinky between the pry bar and drum. EE was seen at the ER diagnosed with a fracture to left pinky & released with restrictions. Restrictions accommodated.
EE was trying to loosen a bolt with a ratchet and socket when the ratchet slipped striking EE in the mouth. EE's front #8 tooth was chipped and EE had a small cut on lower lip. EE was taken to the dentist. EE had temp cap installed, 2 stitches to lip and given a full duty release to work.
Employee was using a pry bar to move a lower sheave wheel, when the pry bar slipped, and EE fell backwards onto the drill pipe container (on the buggy rig) and sustained 2 lacerations to the right forearm, as EE fell. EE was examined at the local hospital and received 5 staples and released to full duty.
Employee was working on shim on dipper they placed a bar between the latch lever and H-block. Either the bar, latch lever or H-block shifted creating a pinch point. The employees right ring finger was caught in the pinch point. Employee was taken to ER, they were released with restrictions and referred to Ortho for further evaluation.
Maintenance mechanic called out to work on the east starter sheet machine, as it had jumped a tooth on the up ender outside the chain sprocket. The mechanic used a pry bar for leverage to check the chain tension. The bar slipped catching finger between the bar and the chain sprocket. EE was seen at ER. X-ray showed a fracture to middle finger tip, EE was referred to ortho doctor.
EE was attempting to tighten a bolt with a pipe wrench. EE positioned self in a low stance (kneeling). EE started to pull the wrench causing it to come loose on the fitting and swivel around smashing thumb between the wrench handle and a steel guard plate. EE was taken to ER given four stitches, diagnosed with fractured thumb and taken off work until seen by Ortho.
Tightening pivot pin with wrench. Wrench slipped off and struck face.
EE draining oil filter removed from equip. He set filter in container allowing fluid to fall in a secondary bucket. Draining oil was blowing on everything due to extreme winds. EE tried to prevent oil from blowning everywhere by placing drain tube from the collection drain to 2nd bucket. He cut a piece of 1" vinyl tube with pocketknife and cut his right middle finger.
Employee was using razor blade without a holder to scrape wax from a mold. He was not wearing gloves and was drawing blade toward himself when he cut his left thumb.
Employee smashed middle finger on left hand when tooling shifted.
Employee was guiding pipe that was being pulled by an ATV using a digging bar. The bar began to slip and he stepped over the pipe. The bar dislodged and the pipe hit his right knee/leg.
EE felt something go into his eye while he was using a cutting saw. EE rubbed his eye thinking it would just come out. EE later noticed that the irritation did not cease, he went to look at his eye and noticed a small blemish on his eye. EE was taken into the ER where he was prescibed eye drops and referred to an eye doctor. He was taken off work til 11/12/07.
Employee was removing the nuts and bolts to the cutting edges on a dozer. When his co-worker removed the last bolt, the cutting edge fell to the ground striking the employee in the left foot. Employee was taken to the ER and taken off work with a fracture to his left foot. Ongoing.
While cutting a piece of 1 plate for a bed cable hook, EE attempted to remove the template when the EE'S hand got caught between template & saw horse causing injury to left hand. EE taken to ER and taken off work for 1 day.
Employee was changing a flat tire on a flatbed utility trailer using a Hi-Lift (handyman) jack. While lowering the trailer on the jack, the employee momentarily let go of the jack handle causing the handle to spring back, catching and smashing the employee's hand between the handle and the body of the jack (fractured bone in thumb).
WHILE INSTALLING A BOLT ON THE MAN HOLE COVER TO THE C FILTER, EE'S HAND GOT CAUGHT BETWEEN THE NUT & THE PLATE THAT WAS BEING BOLTED DOWN, PINCHING THE PALM OF HIS LEFT HAND. THE EE WAS TAKEN IN FOR TREATMENT REC'D 2 STITCHES & RELEASE TO WORK.
EE WAS PULLING CABLE AT THE 22 SHOVEL WITH CABLE TONGS. THE TONGS SLIPPED OFF WHEN HE WAS PULLING AND HE FELL LANDING ON A LARGE ROCK WITH HIS LEFT UPPER LEG. IT HAD JUST FINISHED RAINING AND IT WAS WET AND MUDDY. HE FINISHED THE SHIFT, BUT WHEN HE CAME IN THE NEXT MORNING COMPLAINING OF PAIN, TAKEN TO THE ER. DOCTOR DIAGNOSED WITH A DEEP CONTUSION AND TO STAY OFF WORK UNTIL 8/9.
EE received injury to hand when trying to catch bracket that fell out of bench vise. A mechanic was using an easy out to extract a bolt that had broken off in bracket & when it slipped out of vise, EE reached out to catch it. The tap punctured palm of rt hand causing immed. swelling. EE taken to hospital, referred to specialist to check for bone or tendon damage-none
WITH CATHODE UNLOADING TABLE IN THE UP POSITION, USING A HANGER BAR TO PULL BARS TO THE END OF THE LOOPS, HE PULLED SOUTH SIDE HARS THEN WALKED ACROSS TO PULL THE NORTH SIDE. WHILE WALKING ACROSS, HE SLIPPED ON THE TIRE RAMP, CAUGHT HIMSELF WITH HIS LEFT HAND ON TABLE, BUT HIS RIGHT HAND MOMENTUM WITH BAR CAME DOWN ON LEFT HAND, SMASHING THE END OF HIS LEFT PINKY FINGER
EE WAS IN THE PROCESS OF LOADING A WHISTLER INTO A LAUNCHER (USED TO HAZE BIRDS). THE LAUNCHER MISFIRED AND THE WHISTLER HIT THE PALM OF EE'S RIGHT HAND, CAUSING 2ND DEGREE BURNS. EE WAS TAKEN OFF WORK - INCIDENT ONGOING.
Finger became entrapped between nut adjustment and ear backup, while breaking pipe apart.
EE WAS MANUALLY PULLING CABLE WITH TONGS. THE TONGS SLIPPED, SHE FELL AGAINST A PICKUP THEN FORWARD ON ALL FOURS
WHILE CUTTING ELECTRICAL TAPE, HOLDING A ROLL OF CABLE, THE KNIFE CUT UNEXPECTEDLY EASY THROUGH ELECTRICAL TAPE.
EMPLOYEE WAS SERVICING A VEHICLE AT THE LUBE SHOP. HE WAS REMOVING THE OIL PAN DRAIN PLUG WITH A COMBINATION WRENCH. WHEN THE PLUG BROKE LOOSE HIS LEFT HAND STRUCK THE CROSS MEMBER ON THE VEHI CLE AND HE CUT HIS LEFT LITTLE FINGER ON THE SHARP EDGE OF THE CROSS MEMBER.
EMPLOYEE WAS REMOVING THE INSIDE DUAL ON A SMALL DUMP TRUCK WHEN HIS LEFT THUMB WAS CAUGHT BETWEEN THE SOCKET AND THE HUB OF THE VEHICLE. AN EXTENSION ON THE SOCKET DRIVE IS SUPPOSED TO BE US ED FOR THIS TASK, BUT WAS NOT.
