Employee was assisting with cone liner change. Employee was tasked with holding wrench while other employee struck it with a hammer. As the employee struck the wrench, the wrench bent downward, smashing the injured employee's finger between the wrench and the cone.

The miner was using a bar to clear out the screen chute when a large rock struck the bar and caused it to hit the miner in the face. Lost time accident as of 1/23/2020.

The employee was using a large bar to knock a large rock out of the cone crusher. The employee was putting a lot of force behind the bar, when the rock suddenly moved, pinching EE's finger between the bar and the side of the cone. Lost time as of 8/2.

Employee was removing rocks from the grizzly bars at the feeder with a sledge hammer. While doing this, the employee felt a pain in back. Injury reported 1/9/19 and lost time as of 1/10/19.

The injured employee was working with another employee, loosening nuts on a jaw motor to adjust it. The injured employee was leaning forward, pushing on a wrench when the wrench slipped. This caused the employee to be forced forward over the side of the jaw frame where the employee fell on the dirt. The employee was about 5 feet off of the ground.

The miner was using a pick ax to dig up buried SO cord and wire. The miner felt soreness in back after work. EE woke up the next day and felt more sore. Was diagnosed with a back strain. Lost time as of 2/7/18.

The employee was using a two-handed sledgehammer to move material in a feeder by striking the sides of the feeder. As the employee swung the sledgehammer over his head, he felt a "pop" in his left shoulder and released the sledgehammer, which then struck the same shoulder (left). The employee has scheduled an MRI to determine the extent of the damage (an x-ray revealed no damage).
