Showing all 27Employee was using a pry bar, when it got loose from EE's hands between the bar & structure bruising it. No further injury nor prescriptions but doctor did issue restriction to not use due to swelling.
Employee was unplugging chute when employee fell injuring back and ribs.
Temporary employee was cutting the ANFO hose on the powder truck. The knife slipped cutting the employee's arm. The employee was taken to the emergency room where the employee received 11 staples. The knife was being pulled toward employee's body instead of away.
Employee hit C-channel with wire snips to nudge it into place. Slipped and jammed hand onto metal clip. Has a puncture in right hand. Had medical treatment and returned to work on the Monday following with doctor's release.
Employee suffered laceration to left, middle and ring fingers, as a pry bar pinched the fingers between the pry bar and housing.
Attempting to remove sheave on crusher while hammering wedge, wedge flew out and hit right hand.

Employee was working with the plant mechanic and was repairing a conveyor belt at the crusher. The employee was making a straight cut of the 30 inch belt when the razor knife slipped and cut his leg slightly above the left knee. Later that evening, the employee, called the supervisor and said that he wanted to go to the ER because of the pain in his leg. He received two stitches.
Employee #1 was holding the slug wrench in place while employee #2 was hitting wrench with a hammer. The hammer hit employee #1's left middle finger. Employee was diagnosed with a fracture to the tip of finger. He received a tetanus shot, glue for the laceration, a splint and an antibiotic prescription.
Employee was hitting the clogged chute and pinched his ring finger between the moving sledge hammer handle and the vibrator. He felt the impact but thought it insignificant. A few minutes later he noticed some blood on his finger. No problems with the injury until 12/28/10.
Laborer was driving wood stakes into the ground with a sledge hammer and hurt his back.
Employee was using a pipe wrench to tighten a fitting next to the 02 analyizer. The wrench slipped off the pipe causing the employee's right arm to contact the flu gas discharge pipe which was hot. The employee sufferd 2nd degree burns to his arm and hand.
Injured was raking a piece of coating from kiln grates when a piece of lime entered his eye.
EE repairing rubber hose at Agg plant using utility knife to cut the hose, the blade broke and knife lacarated top finger on right hand. EE stated he was wearing gloves. This resulated in MSHA reportable (stiches) no lost time accident.
Employee working with another mechanic to replace a screen motor at lime processing plant, employee placed his left index finger in a bolt hole to check the alignment. The other employee was prying on base plate, the base plate shifted cutting off end of left index finger.
EE was assisting with clearing a blockage at Gryatory crusher and feed chute when a slab of rock slid down the chute striking right elbow. This resulted in an MSHA reportable accident because of one day restricted duty.
EE was removing wet material blocking an airlock using a 2.5' pry bar (wearing heavy gloves). While gouging out the material his left ring finger struck against a connection flange. This resulted in sutures which made it a reportable, no lost time accident.
Reparing water line with two shop EE's who were using a bar (~15lbs.)to raise line, the bar slipped and struck the EE on the right shoulder. The foreman was aware but EE did not think he needed medical attention. Monday when EE reported to work with pain in right should was sent to medical treatment. This resulted in MSHA reportable.
Closing lid on pneumatic trailer using a 8' pole from catwalk above loadout chute. Leaned over the handrail to reach lid, it was stuck requiring extra force. He felt a slight pull in lower back but continued to work. Reported injury on 4/07/06 but did not seek medical attention until 4/11/06.
EE was tightening bolt with a rachet, the socket broke, EE fell backwards landing on his left side causing lumbar strain. This is a restricted duty report.
EE was assisting with the removal for the fork attachment on skid steer. EE was using a 5' section of drill steel to break free/striking at the bucket release lever. EE missed the release with the bar overstressing his R shoulder/upper arm, resulting in a MSHA Reportable Restricted Duty Injury.
REMOVING A SEAT BELT FROM CAT LOADER, STANDING BENT AT WAIST INSIDE CAB USING RIGHT ARM FOR SUPPORT WHILE USING LEFT ARM TO LOOSEN BOLT. EE USING RATCHET WRENCH APPLYING TORQUE TO REMOVE BOLT WHEN WRENCH SLIPPED. THE WRENCH STRUCK THE EMPLOYEE IN THE LEFT EYE GOING UNDER SAFETY GLASSES.
EE WAS WORKING TO REMOVE BLOCKAGE FROM A #1 SCREEN CHUTE AT THE MAIN CRUSHING. HE WAS USING A 12' CLEANING ROD THROUGH AN ACCESS PANEL. WHEN THEBLOCKED MATERIAL WAS DISLODGED, IT GAVE WAY AND PUSHED THE ROD THROUGH HIS HANDS AND INTO HIS STERNUM.
EMPLOYEES WERE ATTEMPTING TO MOVE AN ID FAN MOTOR. ONE WAS HOLDING A 2' LONG 4X4 WOODEN POST WHILE KNEELING ON THE FLOOR. ANOTHER WAS USING A SLEDGE HAMMER TO ALIGN THE MOTOR WITH THE ID FAN. A 10'X4" HEAT SHIELD WEIGHING OVER 100 POUNDS FELL FROM THE IMPACT ONTO THE FIRST EMPLOYEE'S LEG.
EMPLOYEE WAS ATTEMPTING TO CUT A LOCK OFF OF HIS LOCKER WITH A HACK SAW. THE SAW SLIPPED AND CUT HIS LEFT THUMB.
