The employee bumped into the chute gate valve an activated the chute gate, which struck a scaling bar they were using to relieve muck. The end of the bar caught their left cheek and cut them.
While scaling, loose material fell from the hanging wall and landed on the scaling bar. The scaling bar was pulled from the EE's hand causing the EE's arm to get pushed back and twist. This resulted in a torn ligament in the left bicep. The EE was placed on restricted duty by the physician.
While scaling, loose material fell from the rib and rolled over the EE's ankle causing a sprain/strain of the ankle. The EE was placed on restricted duty by the physician.
Miner was baring a rock out of a chute. Rock came loose knocking scaling bar out of EE's hands and hitting EE on the side of the jaw.
Miner was barring out a chute of the 3700-054 raise. The material came down and hit the bar. The bar then hit the miner in the left ear.
4900-310 switch - move supplies by timber truck. Derailed timber truck; had to use draw bar to lift each side back on track. Felt "pop" to left shoulder. Employee did not report accident till Wednesday, October 27th. To clinic Monday November 1st.
4900-177 barring out bolts stuck in chute, Rock came loose, came out over gate hitting right thigh.
5200-347 barring rock out of chute, felt sharp pain in groin, pain went slowly away, grizzling on 3700, moved piece of steel, pain came back.
5200-164 - Using powder knife to cut water hose; knife slipped and went into left thigh.
5200-346 Chute - barring overhead in chute, felt pain in right shoulder.
3400-048 Raising crib smashed left index finger with ax driving 60 penny nail above employee's head
5200 Grizzly - using double jack to break rocks on top of grizzly; felt strong pain in right hand.
5200 Grizzly - using double jack, something popped in left forearm.
3400-49RS install raise gate 20' down from top; nail lagging to side for gate to stop on when you close it. Had to hold lagging with left hand to nail in place, hit back of hand with poll of axe. Fracture middle left finger.

Galena Mill - #2 belt scraper, tighten scraper, bar used to tighten scraper slipped employee slipped fell into metal ladder rung cutting eyebrow above right eye. 4 sutures
Knock rock out of drill hole using rock bolt and double jack, hit rock bolt with double jack, smashed left little finger between rock bolt and double jack
Miner pried rock off muck pile to move 15 hp. slusher up raise, rock slid down and hit miner in left shin.

Clearing grizzly of muck, swung double jack - rock broke, chunk came up hitting miner in mouth breaking upper incisor tooth
In 3000 Level Grizzly, using double jack to break rocks, piece of rock flew up and broke front tooth.
In 3000-079, hit large rock with smaller rock on top; smaller rock came back after being hit with double jack hitting miner in right eye. Miner was wearing safety glasses at time of accident.
Employee was attempting to remove a plug from a pump. There was a pipe in close proximity and slightly above the pump. As he was attempting to loosen the plug the wrench slipped and his chin made contact with the pipe causing his jaws to slam together. This resulted in a chip in Tooth #8 and piece of tooth #14 was broken off.
The employee was attempting to bar a large rock that was stuck in a muck chute. As he pried on the rock he felt a sharp pain in his back. He was diagnosed with a back strain/sprain.
The employee was breaking rock on the grizzly with a double jack. When he made contact with the rock a small shard of broken rock was ejected and it struck him on the wrist between the cuff of his glove and the Kevlar arm guard that he was wearing. It caused a small puncture wound that nicked a vein causing significant bleeding. He received one suture.
Employee was using a hack saw to cut a 3" plastic pvc pipe in an area with a whiz-bang set up for ventilation. While cutting the pipe some debris was blown in behind the safety glasses and became stuck in the right eye. The debris was washed out underground.
An employee was breaking rock on the grizzly when a large shard of rock flew up and struck his glasses knocking them off of his face. At the same time a tiny fragment of rock became embedded in his eye.
The employee was breaking oversized rock on a grizzly using an 8 pound double jack. He swung and struck the rock when the handle of the double jack broke on impact. He said he felt a pop in his elbow when this occurred and he also noticed pain. The doctor diagnosed this injury as a tendon strain and placed him on restricted duty.
The employee was breaking a large rock on the grizzly with a doublejack when a shard of rock flew back and struck him on the left forearm. He was wearing Kevlar sleeves but swinging the doublejack had caused one of the sleeves to slide down which exposed a portion of his arm.
The employee used a rap wrench instead of a long handled tool to break the bands on a bundle of timber and when the band broke one end recoiled and contacted his arm causing a laceration which required sutures.
EE is a miner trainee and was observing a slushing operation. The trainer was using light signals to direct the operator. He had been warned not to get too close to the chain that was supporting a Sheave block for the slusher cable but he walked into the hazard zone just as the chain broke which caused one end of the chain to recoil and strike him causing a contusion.
The employee was pulling muck from a shaft pocket when the material hung up due to a large rock bridging the chute opening. He was using a scaling bar to dislodge the rock and when it broke loose it struck the bar and cantilevered it upward striking the employee in the mouth.
Hoisting a slusher bucket, bucket rolled and hit employee on the wrist.
Employee was using a double jack to beat boulder in Grizzly. He hit the cat walk on back swing, lost his balance, swung down, and double jack struck him in the left knee.
EE was swinging sledge hammer, trying to break nut on cone head loose, felt "R" knee pop. EE went in and had MRI taken, resulting in surgery on Monday, May 23rd. EE did not lose any time or work restriction until May 23rd.
4000-259 I Drift. EE was cutting his haul back cable on slusher bucket 8' from face, pulled cable tight on top of bucket to get it into cutter on top of bucket. Held cutter w/left hand and hit cable cutter with sledge hammer in right hand. After hitting the cutter 5-6 times, missed cable cutter, hitting his left hand smashing left ringer finger. 3-sutures.
EE was cutting a wrench holder out of a piece of air hose, slipped and cut left hand. Laceration to "L" hand.
Employee was working on the grizzly breaking boulders with a double jack. Employee was hot and sweaty causing safety glasses to slide down his nose at the same time he swung the double jack. A rock chunk flew up from striking the employees left eye.
Employee was barring down in the work heading when he felt a sharp pain and cramping in his lower back.

Employee was barring on muck chute. Boulder came down and hit bar causing employee to lose grip on bar striking him above the right eye.
EE was using knife to open a box, EE slipped using the knife causing a laceration to his left hand. EE needed sutures.

Employee was breaking boulder on the grizzly when a piece about the size of a baseball broke off striking him just below the knee cap.

THE EMPLOYEE WAS USING A PRY BAR TO ADJUST A ROLLER RACK ON A SLUSHER WHEN BAR SLIPPED AND STRUCK HIM IN THE FOREHEAD.
THE EE WAS IN THE PROCESS OF TORQUING A PLANETARY OIL FILL PLUG ON A L.H.D. WHEN HE FELT SOMETHING PULL IN HIS LOWER BACK.
EE WAS REMOVING A TYWRAP THAT WAS SECURING A SET OF GASKETS WITH A UTILITY KNIFE WHEN THE KNIFE SLIPPED & CUT HIMSELF.
THE EMPLOYEE WAS CLEANING DOWN A RAISE WHEN A FOREIGN OBJECT ENTERED HIS EYE. HE WAS WEARING SAFETY GLASSES.
EE WAS REPAIRING SHAFT TIMBER, WHEN HE PRIED UP ON THE BAR, HE FELT A PAIN IN HIS LOWER BACK.
