Worker was helping 2 vendor mechanics disassemble tooling used to install a hard bar pin in a CAT D10R Dozer (Unit #6013). The tooling was "bound," & each of the individuals began taking turns using a small sledge hammer to free the tooling. EE on his 2nd swing freed the tooling, but a piece of it (most likely a socket) hit him above his rt eyebrow requiring 12 stitches.

Employee was tightening a bolt on the cutting edges on the Rubber Tire Dozer when a piece of slag broke loose flying under his glasses and into his right eye. The injured employee was placed on modified work for the remainder of his shift and released to work in the afternoon of the following day. There was one day of modified work and no lost days

Mobile Maintenance Mechanic was changing GET on loader using steel hammer. A piece of steel broke off of the GET or hammer and hit the mechanic in the leg. Injured employee received medial aid treatment (stitches) at local ER. No lost days or restricted work. Employee released to full duty same day.

Employee was opening waste rock sample buckets and cut left ring finger with a utility knife. Employee received 4 stitches to treat wound.

Removing ground and gauging tools. EE stood in bucket and hit the ground and gaugind tools with hammer. A piece of metal broke off and hit employee in upper right thigh.

Two mechanics were removing a pressure valve from 455 hydraulic system. One ee was pulling on the end wrench and could not remove. The other ee applied pressure with his foot and smacked ee in lip. (1 7/8 in wrench)
