Miner was attempting to move conveyor with chain slings and excavator. Chain slipped on conveyor causing it to swing. Miner attempted to stop swinging conveyor and was pinned between suspended conveyor and adjacent fixed conveyor.

Working under crusher motor. Was loosening motor base bolts when open end wrench slipped off and struck EE in the upper lip. Received stitch in upper lip.
WHILE USING A 4-LB. HAMMER TO STRAIGHTEN OUT THE ANGLE IRON ON A 70-FT. PORTABLE CONVEYOR, EE MISSED THE ANGLE AND STRUCK HIS LEFT THUMB. 5 STITCHES WERE REQUIRED TO CLOSE THE WOUND.

IN AN ATTEMPT TO UNPLUG THE NORTH CHUTE A LARGE CHUNK FO ROCK FELL FROM A SIDE HITTING THE DI-N-BAR, CAUSING THE BAR TO SLIP FROM HANDS AND HIT EE IN THE JAW. SUFFERED A BROKEN TOOTH ON LOWER RT. JAWLINE.
