Showing all 8Employee was cutting a gum rubber boot with a folding knife and no cut gloves on. The employee was cutting in a upward motion and knife blade turned in EE's hand, at this point the knife blade closed on EE's finger.
Employee was cutting the bottom out of a supersack of raw material. The bag bulged out when cut & pinned his left arm & shoulder between the hopper and bottom of sack. He then jerked his arm out of the pinch point.
Employee was placing .sprocket on the elevator shaft on the roller mill. His knee came in contact the back elevator's door flange. This caused pain and swelling in the right knee.

IT BECAME NECESSARY TO REMOVE/RELOCATE 26" PVC WATERLINE FOR TRUCK ROUTE SO SAND COULD BE REMOVED FROM CEILING PILE. THE PIPE SNAPPED LIKE A BOWAND IT'S ENDS CAUGHT MY FINGER. 9 STITCHES REQUI RED.
