EE was using a sledge hammer to hit the side of a chute in an attempt to dislodge the material in the chute. As he swung the hammer, his right index finger made contact with the door bracket, causing a laceration that required stitches.

Showing all 7While removing a taper lock with a chisel, ee felt a slight irritation in the inside corner of his left eye. As time went on the irritation worsened. Looking in the mirror ee could see a little black dot in the eye.
While using rod to clear jam in sample port, rod penetrated blockage and moved unexpectedly into opening causing employees hand to come in contact with end of sample pipe causing laceration.
Ee was lifting drive sprockets and drive chain to top of elevator with rope. Shoulder popped while lifting. This employee complained of this injury in January 2006, but was seen and released at that time. This is a revisit for that injury which required no treatment.
EMPLOYEE WAS CUTTING ROPES THAT BANDED QUANTITIES OF LARGE PRODUCT BAGS AND THE KNIFE SLIPPED CUTTING HIS LEFT HAND.
EE WAS INSIDE INSPECTION DOOR OF #2 DRYER BREAKING LOOSE CHOKED MATERIAL FROM UPPER LEVELS OF THE DRYER. A LARGE HARD PIECE OF MATERIAL BROKE LOOSE & FELL, HITTING THE CROW BAR & HIS HAND, PUS HING HIS HAND AGAINST THE DRYER WALL.
EMPLOYEE WAS SHOVELING DIRT UNDERNEATH DRY MILL WHEN HE JAMMED HIS ELBOW ON HARD MATERIAL.
