During routine maintenance of the secondary slitter, the mechanic was adjusting the cutting wheels. While loosening the set screw, the employee's right hand thumb area made contact with the cutting blade, resulting in a laceration.

EE was working on cleaning out mixer. EE picked up a pry bar to break loose hardened material from mixer flights. While doing so, the ee hit the buildup of material and the force from pry bar hit EE in the face near EE's eyebrow / forehead area. EE suffered a laceration and needed stitches.

EE was conducting work in the mill underflow tubs by chipping away steel ball chips from the grating when a piece of metal went into their eye and began causing discomfort. EE was taken to get medical treatment and was prescribed an antibiotic.

EE was in the process of changing a pin on the incher. When EE tried to remove it by hitting it with a hammer, a piece of the hammer metal came off of the pin and went into EE's leg. Embedded piece was not removed.

An employee was assisting another employee w/ removing a zip tie off a fuel can nozzle. EE was holding the can while a second employee was cutting the tie w/ a knife. Knife cut thru the tie & the momentum continued forward causing a laceration to the top side of the EE's thumb & index finger on the webbing. EE had stitches & issued restrictions.

Employees were changing wall liners on 9-ter feeder belt. An employee was using a prybar to remove old liners. As EE was unbolting the first bolt, the two liners on top fell and hit EE's hand. Three employees were attempting to unplug the drop chute on 2-1 & 2-2 hammermill. The EE was hitting the chute about 10 times with a pry-bar. They hit their left pinky finger against a piece of angle iron that is attached to the hammermill drop chute. The EE noticed blood on the piping. They received a laceration to the finger. EE was given 5 stitches and restrictions.

EE was cutting cardboard with a Stanley knife to cover the cooler inlet into the change room. EE used a straight edge on the concrete floor to cut against. The knife jumped over the straight edge and lacerated EE's left thumb that was holding the straight edge still. EE was issued steri-strips.

EE was on drill deck assisting with a broken steel. EE was using a 1/2 bolt that was 7 inches long to knock out piece of steel. The cut steel broke and struck finger that was in a pinch point. The EE fractured the finger tip.

EE was installing Cisco switches in the second floor of the Mill MCC. EE was trimming zip ties with a 1" utility knife when EE lost control of the knife and it made contact with left forearm. Causing a small gash. It required 5 stiches.

EE was working on removing old bolts from a flange with a bull pin and hammer. One of the hammer strikes made light contact with the bull pin. EE felt something on hand. EE began to bleed lightly on left hand. EE had sliver of metal go in to the flesh between the knuckles. Sent to clinic to have sliver removed.

EE was working on A3 Conveyor replacing saddle. EE began to fillet the cables on the splice when the blade slipped and punctured through chaps and jeans. The blade went into kneecap causing a laceration. EE was given 5 stitches to close the wound.

EE was using a pry bar to determine the cause of a jammed table when the energy was released from the table causing the pry bar to strike the employee on the bridge of the nose and face. Security personnel came upon an investigation of a private citizen that had come on to FMI property and committed suicide. Unknown deceased female had shot herself on property by railroad track. EE was working on changing return idlers on A2 conveyor. They used two come-a-longs & a spreader bar to lift the belt so the idler could be removed & replaced. The hook on the main body of one of the come-a-longs opened up & slipped out of the hole where it was attached. It struck the employee on his left ear causing a laceration. EE received stitches.

EE was working on 8th floor of Roaster building on lighting project. Moly operations were poking the roaster. EE smelled rotten egg odor & saw white smoke puff. EE complained of inhalation of possible H2S. Transported to medical facility to be treated. Administered nebulizers at hospital. EE continues to complain of shortness of breath; not fit for duty.

While employee was using a pipe wrench to loosen bolts he felt a slight pull in his lower back. Employee was taken to clinic for lower back pain. Employee was diagnosed with a Lumbosacral Strain. Employee was placed on restricted work schedule (light duty). Employee did not report for light duty for 3 days due to pain in lower lumbar.

EE was tightening a nut that secures the ceramic plate to the drum. The ratchet spanner slipped off the nut and cut the employee's wrist on a stainless steel edge. The employee sustained a laceration to his wrist and stitches were required.

EE was working on the north line installing saddles on the bleeders, came on a saddle that was plugged. Was using a small pry bar to unplug saddle drain discharge line as he was cleaning the saddle the EE's right pinky finger came into contact with the discharge line nipple and small pry bar. This caused a laceration and fracture to EE's pinky finger.

EE was working to clear the chutes between the centrifuges and the vibrating feeder by using a scraping tool in an upward and downward stroking motion. The EE's hand came into contact with the edge of the chute resulting in a deep cut along the knuckle of the EE's right ring finger. Four stitches were required to close the laceration.

EE was cutting a roller bearing apart to perform a failure analysis. He cut thru bearing approx. 95% of the outer race with cutting disc & was using a hammer & chisel to finish separating the material. As he struck the chisel he felt pain in left hand. Piece of bearing had broken free & gone thru glove cutting index finger. Occurred on 6/12/12-specialist removed piece on 7/12/12

EE was finishing with an Agitator motor change. While tightening final bolts on motor mounting bracket his ratchet broke. The forward momentum caused him to slip forward hitting his mouth on the motor mounting plate. EE sustained a chipped tooth and a small laceration to his lip.

Employee was replacing timbers in basement of tankhouse. He was using a hammer to put timbers in place. Debris would fall when he would hit timber. Employee removed goggles and face shield and as he did so debris fell into his right eye. Flushed with water and sterile eye wash to remove particle. Employee taken to clinic was prescribed antibotic and eye drops.

Employee was working on the #43 Shovel removing the Equalizer on the left side. He was a 12lb. Hammer to beat the pin out. As he hit pin the pin shifted causing the inside piece to break free, falling on his left thigh. He received a puncture wound 1¼'X1½' deep. He received 10 stiches on his leg and was released back to work.

A mechanic was attempting to adjust the governor on a welding machine. As the mechanic was loosening a nut on the governor linkage, the wrench slipped off the nut and the fan struck the mechanic's left hand third and fourth fingers.
