A Blasting Operations Technician was opening a bag with a utility knife and cut their leg. This caused a laceration that required stitches. The employee returned to work the next shift.

Employee was working on grader replacing cutting edges. The employee over torqued a bolt causing it to break loose. The bolt hit the underside of employee's hardhat ricochet to employee's forehead striking employee above the right eyebrow, creating a laceration. The laceration needed 5 stitches to close.

The technician was using a tool to hold pressure up on a plow bolt to prevent the bolt from spinning while removing the nut. The tool slipped and the technician's hand contacted the spinning bolt head causing a laceration.

Employee was using a come-along to help install chains on a Liebherr T264 at the mine site. While installing the chain, EE slightly tweaked EE's back. EE continued the remainder of EE's shift until 7:00pm and returned home for the day. The following morning EE came to the site to report soreness in EE's back.

On 9/4/2023, employee was trying to clear a plugged feeder chute with a pry bar, employee's hand slipped off the bar and the tip of a finger struck a rock. Employee did not report the injury until 9/15/2023 and sought medical treatment on 9/16/2023.

Technician 1 struck GET with hammer found in workshop (hardened steel) whilst technician 2 applied force to GET with a pry bar, on the 3rd strike a metal fragment dislodged from GET and struck Technician 3 in the left arm (inside of elbow) after piercing 3 x layers of work clothing.

The employee was trimming a wooden wedge to plug a leak in a tailing line. The knife cut completely through the wood and lacerated EE's hand. EE received six sutures to repair the wound.

On November 3, 2016 at approximately 10:15 PM employee was sharpening powder knife on the 7700 level at 625 Day Mag. The knife slipped from the sharpening tool and cut right middle and index fingers. Employee was transported to the hospital and received 6 stitches.

The employee was using a scissor lift and boom lift to put up a support structure for an overhead pipe. He and another employee had positioned the beam with a chain hoist. While they were positioning the beam it started to rotate toward him (roll) the employee let go of the chain and tried to stop the beam from rolling when it rolled over onto his hand.

The employee was adjusting a spray nozzle for dust control, to do this, the tie-wrap needed to be cut and replaced. While cutting the tie-wrap, the hose and nozzle started to fall. The employee, using his free hand attempted to grab the hose as he cut through the tie-wrap. The momentum of his hand coming down resulted in a laceration to his left hand.

EE was cleaning steel beams through a floor grating with a "makeshift" L-shape bar. He was knocking scale from a piece of plywood when the bar went through the plywood and the bar hit the grating pinching his fingers between the bar and grating.EE lost his left thumb nail and broke the first joint on his left middle finger.

The employee was hammering set concrete on the grating of a hopper. The hammer head slipped through the grating, pinching the employee's small right finger in between the hammer handle and the grating. This resulted in a hairline fracture and small laceration requiring stitches.

The support structure had been poured 10/27/11 the assigned task for the morning was to strip forms in the process of removing the anchor bolts with a pipe wrench the bolt broke free causing the employee to stepped back causing his ankle to roll.

Injured was replacing teeth on a 980 bucket. A piece of metal entered his right ring finger in the process. Note: This was a first aid case until June 1, 2011, at which time employee elected to have an out-patient procedure performed to relieve pain in that finger.

While removing a bearing from a floatation agitator with a dead blow hammer, the bearing broke free and struck the employee in the back of his right hand. It caused a laceration, approximately 1/2 inch in length. It was cleaned and bandaged, however, it became infected. The employee was sent to the clinic, had the wound cleaned, which lead to the discovery of a damaged tendon.

EE was removing a 6' conveyor belt clamp with another ee. The partner disconnected the jaws on his side first, and dropped the bottom plank. The injured ee's side was not disconnected, so as the bottom plank fell, the jaws on his side acted as a fulcrum. The planks extended past the jaws by about 1", and the ee's thumb was between. His thumb was pinched.

Employee helping to trip out drill steel on raisebore drill, while clamping the break-out tool (clamp) to the steel string, employee got his R middle finger pinched between the 2 parts of the tool when the tool slipped out of position to set the bolt that holds the tool parts together. Resulting in section of the finger tip and finger nail being severed from the finger.
