Employee used a hammer and a prybar to free a 10-12" rock wedged in between C2 chute and belt. Rock shattered, causing a large fragment of the rock to bounce and eject back towards the employee. Rock made contact with employee's forehead above right eye. All proper PPE was worn during the time of incident. Employee received 6 stitches and returned to work.

EE was unplugging transfer hopper when rocks broke free hitting the bar that EE was using to free up material. The bar pushed back and caused EE's left hand to impact the conveyer frame still holding the bar. EE had MRC predator Safety gloves ANSI IMPACT 1, PUN 2, ABR 3. EE was sent to ER for X-ray. EE got 8 stiches w/ antibiotics. No ligament or bone damage.

Employee was using a self-retracting razor knife to replace a sweep on the jaw crusher discharge belt. While cutting the sweep, employee was not wearing cutting gloves and overrode self-retracting action of knife. While cutting, left hand slipped under blade, cutting through regular glove and into left pinky finger - EE was taken to urgent care and received 4 stitches.

Mechanic was hammering to crimp on a battery cable, missed and struck his thumb. After several days worker decided to go to doctor, where x-ray revealed a fracture to thumb. We received notification of fracture at this point (3/28), creating a recordable injury.

EE attempting to clear feeder over scalping screen. He tried to dislodge stones with shovel. When stones let loose, shovel handle struck EE near mouth and nose area. Four stitches required to close cut near mouth.
